🔁 Throwback 🔁

With all the presentations lately this post has been doing the rounds

It’s my go to for jaw reg chat

Wonderful wax work from @soaresdtg and a fit to match!

This was the first case I documented so I didn’t have a lot of shots for each case

Shall I cover more on jaw reg? #impressionclub taken in Woodborough House Dental Practice by @dentistrupert

🎂 Birthday Builds 🎂

The upper imps to go with yesterdays lowers

I’ll be going through these steps at tomorrows lecture for the ITI in Birmingham on Optimal Complete Dentures and Digital Workflows!

Here I’ve built the upper in the usually way with @kulzergb silicone and @zhermack_official alginate

The secondary is all kulzer, note the shine through in the palate suggesting the tray was a little under extended and I had to seat it further at the tissue stop step

We fitted these a while ago and luckily this had no consequence - I could have trimmed this back before the final wash layer

Much like the lower, this is where the photos come in handy to remind ourselves where adjustments may be needed - you could also put it in your notes 📝

Thanks for all the birthday wishes #impressionclub taken in Woodborough House Dental Practice by @dentistrupert

🔽 Lower Partial Primary 🔽

Here’s the opposing arch for the upper complete in the previous post

Another classic lower free end saddle, here we did some composites to stabilise caries and extracted the retained root after taking the impressions

As ever these cases tend to have a lot of resorption and the ridges end up below the dentate tray, without building we have no chance of alginate making it down there

That being said I’m not 100% happy with the lingual extension on that patients right side, it looks short in the putty compared to the other side, but the alginate covering looks similar so perhaps this is anatomical

I could’ve tried adding a little more silicone, either putty or heavy in a gun for better access

However as I am doing a chrome which I will do digitally with an altered cast impression after this is just a baseline and I can easily get this right later on!

If I wasn’t doing the altered cast impression after I would be redoing this!

If you want to learn more about these workflows join Dean and I on dentures in a day, our full day lecture series in November in London or live stream!

Hands on workshops are sold out with new dates planned for the new year

🥾 Another Case Kickstarted 🥾

A new case for a lovely lady with an upper complete and lower partial

As ever it’s all about the build to start, getting as much information as possible to begin allows @sjh_prosthetics to make me the best possible trays

This is why I’m always aiming for over extension to see all the land marks, good and bad!

This three step technique allows me to quickly and easily capture any upper edentulous impression

It’s all about repeatability!

We teach this and much more on our primary impression course (looking forward to getting back to Bath next weekend!) with our final courses for the year already sold out

Dean and I also cover anatomy and impressions as part of our full day lecture series which has availability in person in London and online in November if you want to get a head start on your cases before the new year!

⚙️ Digital Denture Tip ⚙️

Sometimes a quick scan of the existing de tires can be super useful

I’ve used this to communicate issues with dentures to highlight this to the patient

Or in this instance a quick scan can give @sjh_prosthetics a guide on the aesthetics that the patient likes!

Let’s see how it turns out #impressionclub taken in Woodborough House Dental Practice by @dentistrupert

A very proud Woodborough House moment, with Dr Nick Fahey and Dr Rupert Monkhouse together with the dental textbooks ✨

Nick is the author of Guided Surgery - Making Implant Placement Simpler, a clinical textbook drawing on his extensive experience in computer-guided implant dentistry and digital treatment planning 🦷

More recently, Rupert contributed a chapter to Digital Removable Dentures: A True Metamorphosis, exploring the essentials of complete dentures within the modern digital world 📕

We're incredibly proud to have clinicians who not only bring such a depth of knowledge and experience to the care of our patients, but also share that expertise with the wider dental profession 🤍

A fantastic achievement for them both, and a lovely moment to have the two books together at the practice 👏✨

🔑 Consistency is Key 🔑

For me the key to a great primary impression is consistency, I know with these three simple steps I’ll get an acceptable impression

Remember 💭 stock trays have to fit everyone, so they can’t fit anyone!

Using the four tissue stops it helps me gauge how much silicone and where that silicone is needed for the second stage

It also gives me the consistency, I can only place the tray in one way!

With the upper I’m always focussing on seating the posterior first and bringing the anterior forward - this has to the same every time

The second layer allows some border moulding, I aim for premolars backwards

I build the putty above the level of the stops and place the tray in the same way, pressing until the stops reach the palate - this way I get a great seal

Then the alginate is straight forward, but again it must be seated in the same way!

If you want to take your cases to the next level join us at dentures in a day where we cover from impressions through to chromes and immediate dentures

The next day is in London but we are also offering a live stream ticket for those further afield!

⚙️ More Digi-Logue ⚙️

Lower free end saddle chromes are another classic example of the hybrid workflow - combining analogue principles with digital elements to get an optimal result!

I’m a big fan of the altered cast technique - a method of allowing you to balance the support from the hard and soft tissue - excellent for these free end saddles

This is done by taking an impression within the saddles on the metal framework - compressing the soft tissue with the framework at maximum engagement, just like it will be in function

This has the added benefit of also giving us a functional impression but also letting us make our framework off the scan and we love our scanning accuracy for the teeth normally!

My workflow here is as follows:

1️⃣ Analogue ‘built’ primary - I still like an impression here to guide the lab with where they should extend the custom trays to later - you need a model to plan the denture on so it may as well give us soft tissue markers too!

2️⃣ Scanned ‘secondary’ - after relevant tooth preps, in this case rests on the fours and composite cingulum rests on 3-3, we scan aiming for great detail on the teeth and as much length of the ridge as possible

The length of the ridge is important as the chrome will be made on this model and we need a good length of the metal frame over the ridge to support our saddles

3️⃣ Verification - here we have a printed verifier, great for patient consent about where metal may show but also if this fits then your expensive metal should too! If it doesn’t, re scan 🔁

4️⃣ Metal try and altered cast - the lab makes the metal work and adds trays to the saddles, once the fit is confirmed we can carry out the altered cast impression

- light body silicone with no adhesive to check fit, mark shine through with a pencil and adjust. Remember to engage the rest seats with finger pressure and not occlusion

- once adjusted add adhesive and repeated the impressions, with finger pressure, and border moulding

- the lab then sections the model & pours in the stone

5️⃣6️⃣7️⃣8️⃣ 🕺🏻 from this point you can carry on with your try in and your fit

⚙️ Digital Manufacture ⚙️

When we talk about digital dentures we have a number of different ways that they can be made

Including sometimes a combination of the two!

Have a swipe through this multi-caption to explain some of the options available!

Dentures in a day - out full day lecture series releases tomorrow - now with an option to live stream from anywhere around the world!

Here we have a milled base with digital individual teeth from @schottlanderdental

My preference is currently for milled as it has a better long term track record - printing is evolving rapidly though and will be the future!

Benefits of milled over conventional processing include:
- No distortion of your acrylic
- Increased strength
- No movement of teeth in processing
- Milled occlusion is highly accurate

Here @sjh_prosthetics has used the Schottlander digital teeth - the base is designed for the shape of the tooth and they are bonded individually

Whilst these are the most aesthetic teeth in a digital denture that I’ve seen, they aren’t truly digital as they are not designed for the individual case and would need to be adjusted by hand to get them ‘custom’ for the occlusion

The milled base has of course had composites and stains added!

Here is a printed try in, it’s a monoblock shade eg A2

The teeth are stained and the gums painted over

This is excellent for a try in and to allow the patient to take home and try out

Definitive printed dentures are printed in separate base and teeth materials and bonded together

As in the previous slide I’m not there with printed for definitives yet and have only done them for short term temporaries

Another view of the same printed try in

The lower chrome for the case was made analogue on our hybrid model

The teeth were scanned and the chrome made, the altered cast technique was then done to give functionality

The chrome was made in conventional methods - digital methods include milling (typically titanium) or metal ‘printing’

This additive technique can either be selective laser sintering or melting

I’m yet to make a digital chrome but would be keen to see some!

💻 Digital Immediates 🩵

One of my favourite workflows is scan to fit immediate completes - we are doing a lot lately!

This is were digital can really shine ✨

In the consultation I like to scan - with and without any current dentures - to communicate with the patient and lab if needed

What it also allows us to do is make complete dentures to fit for the next visit if suitable - allowing for a super efficient workflow

This is one such case of an upper complete

From the scan we can merge with the photos to plan the case and go straight to milling the complete denture - no impressions and stressing about extracting teeth with alginate 😱

These scans will never be functional and will have areas of under and over extension - plus as with any immediate there’s a lot of estimation from the lab

Because of this I always look to reline with viscogel on the day of surgery - since I’m relining anyway I ask @sjh_prosthetics to leave some of the teeth rather than full socketing

This leaves me a trough of space and saves time adjusting

For these cases I use fit checker from GC to show areas that are too tight (the areas of acrylic shining through) I use a pencil to mark these areas and peel of the silicone to show the areas to adjust

Once it’s passive I will reline with viscogel - remember to ensure it is bitten into occlusion as to not raise the OVD

Top tip 💡 use bite reg on the teeth first to save time on clean up

After 2 months of healing I review again and replace the liner with GC soft

Looking forward to the definitives!
