Isolated from opium in 1804 by Friedrich Sertürner and named after Morpheus, the Greek god of dreams. It was the first alkaloid ever pulled from a plant, and 200+ years later it’s still the yardstick every other opioid gets measured against.

Things worth knowing:
• Mu-opioid agonist. Analgesia, sedation, and the respiratory depression you’re always watching for.
• Releases histamine, so expect itching, flushing, and sometimes a dip in blood pressure.
• IV peak is slower (~20 min) than fentanyl’s. Give it time before you redose.

The usual suspects:
→ Ulnar nerve: among the most common perioperative nerve injuries. Pad the elbow and keep pressure off the cubital tunnel.
→ Brachial plexus: keep arm abduction under 90° and the head neutral.
→ Common peroneal nerve: watch the lateral knee in lithotomy.
→ Eyes in prone: no pressure on the globe. Check them, then check them again.
→ Pressure injuries: heels, sacrum, occiput. Long cases add up.

Positioning is a team sport: surgeon, anesthesia, nursing. Everyone owns it before incision and every time the table moves.

A 2010 BMJ review (Cunningham et al.) looked for proof:
→ Post-tonsillectomy bleeding was about 7% in redheads, the same as controls
→ Redheads reported more bruising, but their coagulation tests were normal
→ No link to clotting problems was found

It’s the quiet backbone of multimodal analgesia. Opioid-sparing, cheap, and on almost every ERAS protocol.

Max single dose: 1 g (15 mg/kg if under 50 kg)
Max daily dose: 4 g from ALL sources
Hepatic disease, alcohol use, frail or elderly: cap it at 2–3 g

Esmolol: cardioselective β1 blockade, onset in about 60 seconds, gone in roughly 10 minutes. Red cell esterases clear it, so pseudocholinesterase deficiency doesn’t matter.

Where it earns its keep:
• Blunting the laryngoscopy heart-rate spike
• Taming intraop tachycardia without a long-term relationship
• Rate control before you commit to something that sticks around
