CAAs and CRNAs both give anesthesia in the operating room. The controversy is not mainly about what happens at the head of the bed. It is about how each profession is trained, where each is allowed to work, and which model hospitals and states choose.
CRNA — Certified Registered Nurse Anesthetist
Starts in nursing: BSN, RN license, typically 1–3 years of critical-care experience, then a doctoral anesthesia program (usually 36+ months). Can practice in all 50 states. In some states they work on a physician-led team; in others they can practice without an anesthesiologist present. Largest anesthesia advanced-practice workforce in the U.S.
CAA — Certified Anesthesiologist Assistant
Starts on a pre-med-style path: bachelor’s degree plus science prerequisites, then a 24–28 month master’s program. Always practices in the anesthesia care team model under an anesthesiologist’s medical direction. Authorized in about two dozen jurisdictions, not nationwide. Smaller workforce, but growing as more states license the role.

ANESTHESIA CARE TEAM — KNOW THE DIFFERENCE

Original post by - @dilaudiddaddy_ … be sure to follow his account, he has a lot of great content!

MD/DO • CRNA • CAA

When you have surgery, the person managing your anesthesia is not interchangeable. Training length, clinical experience, practice authority, and billing all differ.

Here’s the clear breakdown:

Anesthesiologist (MD/DO)
12–13+ years after high school. Medical school + 4-year residency. Directs the care team. Independent practice in all 50 states.

CRNA (Nurse Anesthetist)
~8 years total. BSN + ICU experience + DNP/DNAP. Practice authority varies by state.

CAA (Anesthesiologist Assistant)
6–7 years total. Master’s-level training. Works only as part of a physician-directed care team.

Worth noting: When medically directed by an anesthesiologist, both a CRNA and a CAA perform the same intraoperative role and bill the same (QX at 50%). The differences appear in independent practice authority and state-by-state scope rules.

Save this so you know exactly who is responsible for your care.

Credit where it’s due. @dilaudiddaddy_ highlights an important idea: improving workflow and reducing unnecessary delays can absolutely increase throughput and create value. On this, we agree!

Where I have a different view is the economic impact.

The operating room cost-per-minute literature is one of the most frequently misunderstood concepts in healthcare finance. The authors themselves caution against using these figures without understanding how they were derived. An average accounting cost is not the same as realized economic savings.

My goal isn’t to criticize the creator. It’s to encourage a quality conversation for how we interpret research and apply it to real-world decision making.

In leadership, the difference between a good decision and a bad one is often understanding what the numbers actually mean, not simply being away of them.
