📘 1. Clinical Snapshot & Buzzwords

DVT = Deep Venous Thrombosis

	•	Unilateral leg pain & swelling
	•	Calf warmth
	•	Tenderness
	•	Erythema

💡 2. Key Definitions & Mnemonic

Virchow Triad 🧠

Venous stasis
Trauma → endothelial injury
Hypercoagulability

	•	Stasis: immobility, prolonged bed rest, long flights, obesity
	•	Endothelial injury: surgery, trauma
	•	Hypercoagulability: malignancy, pregnancy, OCPs, thrombophilia

Stasis + Injury + Hypercoagulability = DVT

🧬 3. Pathophysiology

🩺 4. Diagnostic Approach

D-dimer
	•	Sensitive but not specific

📌 Step 2 trap:
D-dimer ليس specific؛ قد يرتفع أيضًا في infection, maligna by @dr.mnna_24

Wilson disease.

Key Definitions & Mnemonics

Wilson disease = Hepatolenticular degeneration
	•	Autosomal recessive
	•	Liver
	•	Brain
Kayser-Fleischer ring

Copper → Liver + Brain + Eye

Kayser-Fleischer ring
Descemet membrane.

Pathophysiology

Diagnostic Approach

Best initial test: Slit-lamp examination

📌 Most accurate test: ↑ 24-hour urinary copper excretion after penicillamine

Management Overview

First Aid:
	•	Dietary copper restriction

📌 Exam Pearls 🔥🧠

Wilson = young + liver + brain + eye

🔥 مريض <30 سنة + cirrhosis/hepatitis + tremor/ataxia أو psychiatric symptoms → Wilson.

🔥 Kayser-Fleischer ring = copper deposition

🔥 ↓ Ceruloplasmin + ↑ urine copper

🔥 Initial test → Slit lamp

🔥 Most accurate → 24-h urinary copper after penicillamin by @dr.mnna_24

🧬 Pathophysiology

Metabolic Acidosis

Anion Gap:

AG = Na⁺ − (Cl⁻ + HCO₃⁻)
	•	>12 mEq/L → High anion gap metabolic acidosis
	•	8–12 mEq/L → Normal anion gap metabolic acidosis

High AG — MUDPILES
	•	Methanol
	•	Uremia
	•	DKA
	•	Propylene glycol
	•	Iron/INH
	•	Lactic acidosis
	•	Ethylene glycol

3. 🧬 Pathophysiology

4. 🩺 Diagnostic Approach

Initial test

Spirometry/PFTs هو أفضل initial test.

	•	↓ FEV₁
	•	FEV₁/FVC <0.70
	•	↑ RV و TLC
	•	Minimal (<12%) أو no improvement بعد albuterol.

CXR

	•	Hyperinflated lungs
	•	↓ lung markings
	•	Flattened diaphragms
	•	في emphysema: bullae/subpleural blebs.

ABG

	•	Hypoxemia

في uncomplicated hypothyroidism مثل Hashimoto:
➡️ Levothyroxine.

6. 📌 Exam Pearls 🔥🧠
	•	🔥 Most common cause → Hashimoto
	•	🔥 Hashimoto → ↑ TSH, ↓ T4, anti-TPO antibodies
	•	🧠 Primary = TSH goes UP
	•	🧠 Secondary = TSH goes DOWN
	•	📌 Myxedema coma: severe hypothyroidism + ↓ mental status + hypothermia + hypotension + bradycardia + hypoglycemia + hypoventilation.

🧠 Concentric hypertrophy

Pressure overload

• أمثلة: Hypertension و Aortic stenosis

Pressure → Thick wall → Concentric

🧠 Eccentric hypertrophy

Volume overload

Volume → Dilated chamber → Eccentric

Concentric = Thick
Eccentric = Dilated

والـ First Aid يذكر أن hypertrophic cardiomyopathy فيها جدران سميكة مع impaired relaxation/filling، أي diastolic dysfunction by @dr.mnna_24
