Gonioscopy doesn’t have to be intimidating. It’s one of those skills that feels hard… until it doesn’t. The more you practice, the easier it gets. by @dr.valentinalozano
As a glaucoma surgeon with a strong family history of glaucoma, I’ve thought about this more than you might expect. 👀 Drop your guesses below before you read mine. by @dr.valentinalozano
One of the best gifts we can give our kids is a strong financial foundation. Here are 5 things we’re doing: - 529 plans: Investing early for their future education, with tax-free growth and withdrawals for qualified education expenses. - Investment accounts: Giving their money decades to benefit from compound growth. - Trump Accounts: Eligible children born between 2025 and 2028 can receive a $1,000 government contribution. Don’t leave free money on the table! - custodial Roth IRA: Investing their earned income early so it can grow tax-free for decades! - Life insurance: Making sure our children are financially protected if something happens to either parent. You don’t need to have everything figured out. Starting early, even with small amounts, can make a huge difference. 👇 Comment BABY and I’ll send you the @thewhitecoatinvestor ‘s FREE financial checklist for new parents! A little change in my cataract surgery routine! I recently made the switch to dropless cataract surgery, and I’m excited to share what I’ve learned along the way! Less reliance on postoperative eye drops, a simplified recovery regimen, and hopefully a better experience for our patients. Curious about my technique, medications, and protocol? 💬 Comment “DROPLESS” and I’ll send you my guide sheet! #CataractSurgery #DroplessCataractSurgery #Ophthalmology #EyeSurgery #CataractSurgeon by @dr.valentinalozano
Just because your OCT is GREEN doesn’t mean your patient doesn’t have glaucoma. And just because it’s RED doesn’t mean they do! Ever heard of red disease and green disease? 🔴 Red disease: Your OCT flags an abnormality, but it may be related to myopia, anatomical variation, or segmentation artifacts rather than glaucoma. 🟢 Green disease: Your OCT looks completely normal, but your patient may still have glaucomatous damage! Remember, OCT compares your patient to a normative database. It doesn’t make the diagnosis for you. Look at the optic nerve. Look at the RNFL and ganglion cell maps. Correlate with visual fields. And most importantly, look for progression! Treat the patient, not the colors! Agree or disagree? 👇 by @dr.valentinalozano
Don’t be scared to try new things! If you’re already using a 30G needle to create a paracentesis at the slit lamp when the IOP is elevated, you’re already halfway there. Learning this technique can save your patient a trip to the operating room—and it may be easier than you think! by @dr.valentinalozano

Did you know you can have freckles in the back of your eye? They’re called choroidal nevi — and most are benign. But just like dermatologists monitor certain skin moles for suspicious changes, eye doctors monitor choroidal nevi for features that could suggest transformation into melanoma. A helpful mnemonic is TFSOM-DIM: T — Thickness >2 mm F — Fluid (subretinal) S — Symptoms (vision loss to 20/50 or worse) O — Orange pigment M — Melanoma acoustic hollowness DIM — DIaMeter >5 mm The more suspicious features present, the more closely the lesion needs to be evaluated and monitored. Save this mnemonic for your next fundus exam, and comment “MORE” and I’ll send you a link with more information on choroidal nevus follow-up and monitoring. ——- Educational purposes only. A choroidal nevus should be evaluated based on the complete clinical picture. by @dr.valentinalozano
When a patient presents with an acute angle-closure crisis, I immediately initiate IOP-lowering medications, including oral acetazolamide when appropriate. But if the pressure remains dangerously elevated, I don’t keep waiting. 💉 I perform an anterior chamber paracentesis at the slit lamp. This allows me to rapidly decompress the eye, immediately lower the IOP, and often provide significant symptomatic relief. Why not pilocarpine? At extremely high IOPs, the iris sphincter becomes ischemic, making pilocarpine relatively ineffective. Additionally, pilocarpine can cause anterior rotation of the ciliary body and forward displacement of the lens–iris diaphragm, potentially worsening angle crowding in certain mechanisms of angle closure. Why not go straight to LPI? During an acute attack, the eye is often extremely inflamed, with significant corneal edema that makes laser visualization challenging. Additionally, longstanding or severe angle closure may already have resulted in significant peripheral anterior synechiae (PAS), which an LPI will not reverse. Does this mean I never perform an LPI? Absolutely not! LPI remains an important definitive treatment when pupillary block is the underlying mechanism. I just prefer to stabilize the eye and lower the pressure first when visualization is poor. ⚠️ An AC paracentesis is a temporizing intervention, not definitive treatment, and requires careful patient selection. by @dr.valentinalozano
POV: You’re my new glaucoma patient 👀 This is what mild → moderate → advanced glaucoma can look like on a visual field. The scary part? You may not notice the vision loss until a significant amount is already gone. Glaucoma treatment isn’t about getting lost vision back—it’s about protecting the vision you still have. Early detection matters. Ib: @alex.suh by @dr.valentinalozano
