What does “more anesthesia” mean? 💉🩺

It means a patient may require higher or more frequent doses of anesthetic medications to achieve the same desired effect—such as unconsciousness, amnesia, analgesia, or immobility.

6 factors that can affect anesthetic requirements:

1. Chronic opioid use — tolerance
2. Chronic alcohol use — altered drug response
3. Obesity — altered drug distribution
4. High anxiety/stress — may increase sedative requirements
5. Chronic benzodiazepine use — tolerance
6. Genetic variants — differences in drug metabolism/response

Anesthetic dosing is individualized and guided by the patient’s response and monitoring.

Thoracic Anesthesia 🫁 | Double-Lumen Tube Placement

Thoracic anesthesia is all about precision, planning, and protecting the lungs. 🫁

For procedures requiring one-lung ventilation (OLV), a double-lumen endotracheal tube (DLT) allows the anesthesia provider to selectively ventilate one lung while isolating the operative lung.

🔹 Proper patient positioning & preoxygenation
🔹 Choose the appropriate DLT size
🔹 Careful placement and depth
🔹 Bronchoscopic confirmation is the gold standard for verifying position
🔹 Reconfirm placement after positioning, especially after turning the patient lateral
🔹 Optimize OLV with lung-protective ventilation

The goal isn’t just getting the tube in—it’s getting it positioned correctly, maintaining oxygenation, and safely managing the physiology of one-lung ventilation.

Traveling the world — I make it a personal goal to travel at least once a month, always seeking new destinations, cultures, and experiences that broaden perspective and fuel inspiration.

I encourage anyone—especially registered nurses considering anesthesia school—to go for it. It may be the best decision you’ll ever make. 💉 ANESTHESIA INDUCTION SEQUENCE 🫁

1. Pre-oxygenation – Maximize oxygen reserves.
2. Induction agent – Propofol, etomidate, or ketamine.
3. Analgesia – Opioids when indicated.
4. Paralytic – Rocuronium or succinylcholine when indicated.
5. Airway management – Mask ventilation and/or intubation as appropriate.
6. Confirmation – Verify ETT placement with waveform capnography.
7. Maintenance – Maintain anesthesia and hemodynamic stability.

Credential: Certified Registered Nurse Anesthetist (CRNA)
Professional field: Anesthesiology
Professional title: Nurse anesthetist; “nurse anesthesiologist” is also used by CRNAs and professional organizations, though terminology and acceptance vary.
Physician title: Anesthesiologist

Important distinction: CRNAs and physician anesthesiologists are different professional roles with different educational pathways and credentials. Both provide anesthesia care, and CRNAs may practice independently or within a team depending on the applicable laws and practice setting.

🫁 ENDOTRACHEAL TUBE (ETT) & ANESTHESIA

🔹 Purpose: Secures the airway and facilitates ventilation and oxygenation during general anesthesia.

🔹 Placement: Inserted through the vocal cords into the trachea.

🔹 Confirmation: Continuous waveform capnography (ETCO₂) is the gold standard for confirming tracheal placement, supported by bilateral chest rise and breath sounds.

🔹 Cuff: Helps reduce air leakage and aspiration risk when appropriately inflated.

🔹 Complications: Esophageal intubation, endobronchial intubation, airway trauma, dental injury, and post-extubation sore throat.

🔹 CRNA responsibility: Select the appropriate tube size, secure the airway, confirm placement, monitor ventilation, and ensure safe extubation.

💡 Remember: Oxygenation, ventilation, and continuous monitoring are essential to safe airway management.

* 🩸 Identify and control active bleeding
* 💧 Replace volume appropriately—avoid excessive crystalloid
* 🫁 Optimize oxygenation, MAP & cardiac output
* 🩸 Transfuse PRBCs when clinically indicated
* 🧪 Monitor platelets, fibrinogen, coagulation, Ca²⁺, pH & lactate
* 🚨 Activate MTP for major hemorrhage

Think beyond the number: Blood loss + perfusion + oxygen delivery = the bigger picture.
