Thank you to Dr. Lyssa Daley, DNP, CRNA for being our podium presenter on Obstetric Anesthesia & Advancing Black Maternal Health Equity at the Diversity CRNA 2026 Information Session & Airway Simulation Lab Workshop at Rosalind Franklin University of Medicine & Science in Chicago

Dr. Daley is an alumna of Johns Hopkins University Nurse Anesthesia Program.

During her presentation to marginalized ICU RNs across the country interested in Nurse Anesthesia, Dr. Daley shared a video of a Black father who recently loss his wife during labor due to racial bias, dismissive signs requiring urgent care and informed marginalized critical care nurses on vital statistics in Black Maternal Mortality.

FACT: Black women in the United States are three times more likely than white women to die from pregnancy related causes.

Black mothers have the highest rates of caesarean sections (c-sections) and risking a number of surgical complications, including: preeclampsia, preterm birth deliveries, hemorrhage and maternal death.

These adverse outcomes experienced by Black laboring mothers (parturient) are often link to systemic racism, bias by healthcare professionals, and unequal access to care.

A lesson learned…

In the operating room preparing for a spinal (regional) anesthetic.

in this video: Dr. David Moffor, DNP, CRNA sharing valuable information on Spinal anesthesia medication with proper sedation.

As per Dr. Moffor:

🦴 Spinal Anesthesia + Hip Replacement

Hip replacement doesn’t always require general anesthesia. With spinal anesthesia, medication is placed in the lower back to temporarily numb the lower half of the body while the patient remains breathing on their own.

Add carefully titrated sedation, and many patients can be comfortable and relaxed throughout the procedure.

Precision. Comfort. Safety. Every case, every patient. 💉🦴

Today’s lesson….

Thoracic Anesthesia 🫁 | Double-Lumen Tube Placement by Dr. David Moffor, DNP, CRNA

Thoracic anesthesia is all about precision, planning, and protecting the lungs. 🫁

For procedures requiring one-lung ventilation (OLV), a double-lumen endotracheal tube (DLT) allows the anesthesia provider to selectively ventilate one lung while isolating the operative lung.

🔹 Proper patient positioning & preoxygenation
🔹 Choose the appropriate DLT size
🔹 Careful placement and depth
🔹 Bronchoscopic confirmation is the gold standard for verifying position
🔹 Reconfirm placement after positioning, especially after turning the patient lateral
🔹 Optimize OLV with lung-protective ventilation

The goal isn’t just getting the tube in—it’s getting it positioned correctly, maintaining oxygenation, and safely managing the physiology of one-lung ventilation.

Thoracic anesthesia: where airway management meets advanced physiology. 💉🫁

Full credit to @erinryann. She placed the double lumen tube and Managed the case.👏🏿👏🏿👏🏿.

A word about Propofol: The Foundation of Modern Anesthesia by Dr. David Moffor, DNP, CRNA

Propofol is one of the most commonly used intravenous anesthetic agents in the operating room. Its rapid onset, predictable recovery, and smooth induction make it a favorite among anesthesia professionals for both general anesthesia and procedural sedation.

🧠 Mechanism of Action (MOA)

* Potentiates the GABA-A receptor, the brain’s primary inhibitory neurotransmitter.
* Increases chloride ion influx into neurons, causing hyperpolarization.
* Produces sedation, hypnosis, amnesia, and unconsciousness.
* Provides no analgesia, so pain management requires additional medications.

💉 Benefits of Propofol

* ⚡ Rapid onset: 30–45 seconds
* ⏱️ Short duration: 5–10 minutes after a single bolus
* 😌 Smooth induction and emergence
* 🤢 Excellent antiemetic properties (reduces PONV)
* 🎯 Easy to titrate for the desired depth of anesthesia
* 🧠 Rapid recovery with minimal residual sedation
* 🫀 Decreases cerebral metabolic rate, cerebral blood flow, and intracranial pressure

🏥 Common Clinical Uses

* Induction of general anesthesia
* Maintenance of anesthesia (TIVA)
* Procedural sedation (endoscopy, cardioversion, bronchoscopy)
* ICU sedation for mechanically ventilated patients
* Sedation during monitored anesthesia care (MAC)

⚠️ Important Considerations

* May cause hypotension due to vasodilation and reduced myocardial contractility
* Can produce respiratory depression and apnea
* Pain on injection is common
* Use caution in patients with severe hemodynamic instability
* Continuous infusions require close monitoring by trained anesthesia professionals

🩺 Why Anesthesia Professionals Love Propofol

Propofol offers rapid control, precise titration, and fast recovery, allowing anesthesia providers to quickly adjust the depth of anesthesia while promoting a smooth wake-up. Its reliability and versatility have made it one of the cornerstones of modern anesthetic practice.

“Propofol: Fast asleep. Smooth awakening. Precision in every dose.” 💉💙

A word about Precedex (Dexmedetomidine) in Anesthesia by Dr. David Moffor, DNP, CRNA

Precedex (dexmedetomidine) is a highly selective α₂-adrenergic agonist widely used in anesthesia for its unique ability to provide sedation, anxiolysis, and analgesia with minimal respiratory depression. Unlike many sedatives, patients remain calm, cooperative, and easily arousable, making it an excellent choice for procedural sedation and as an adjunct during general anesthesia.

Mechanism of Action (MOA)

🧠 Activates α₂ receptors in the locus coeruleus of the brainstem.
⬇️ Decreases norepinephrine release, reducing sympathetic outflow.
❤️ Produces:

* Sedation that resembles natural sleep
* Anxiolysis
* Analgesic-sparing effects
* Sympatholysis (reduced heart rate and blood pressure)

Benefits in Anesthesia

💉 Provides smooth, cooperative sedation
🫁 Minimal respiratory depression compared with opioids and propofol
❤️ Blunts sympathetic responses to intubation and surgical stimulation
💊 Reduces anesthetic and opioid requirements
😌 Improves hemodynamic stability in appropriately selected patients
🌙 Promotes smoother emergence with less agitation and delirium
🏥 Commonly used for awake fiberoptic intubation, monitored anesthesia care (MAC), ICU sedation, and enhanced recovery protocols

Clinical Considerations

⚠️ Monitor for bradycardia and hypotension, especially with rapid administration or in hypovolemic patients. Dose adjustments may be needed in elderly or hemodynamically fragile patients.

“Precedex: Calm sedation, stable hemodynamics, and smoother recoveries—an invaluable tool in modern anesthesia.” 🩺💜

Diversity CRNA - Baltimore 2026 conference complete! Another life-changing one in the books! Being on the other side and being able to pour into those who are where I once was is an amazing thing. These conferences changed my life both before and after entering anesthesia school. I am honored to be able to do my part in helping to spark that journey for these critical care nurses pursuing the field of anesthesia. I’m proud of everyone who attended and if you didn’t make this event, it’s OK. There will be more! Keep an eye out future dates!
