The American College of Obstetricians and Gynecologists says if you’re looking to get pregnant, you should aim to stop taking GLP-1s 1-2 months before conception — and stay off them for the course of the pregnancy.

@drpessahpollack discusses the new guidance: “You want to be careful.” by @drpessahpollack

While supplement stacking and multivitamins are becoming increasingly popular for their supposed benefits, overcorrection can often lead to toxic or unabsorbable levels of nutrients such as vitamin D, calcium, and zinc 💊

Ultimately, a well-rounded diet is the best way to get all your recommended nutrients at appropriate dosages. Log your meals and calculate your average nutrient intake before you consider adding supplements.

Consult your clinician to create a health plan and tailor supplement recommendations to you, your medical issues, and potential dietary limitations. ***And always bring a list of any supplements you are taking to your visit 📝

When my patients ask me for blood sugar management strategies, one they often don’t expect to hear is taking short bursts of activity throughout the day, particularly after eating 👣

Studies show even short walks after eating can regulate blood sugar spikes that naturally occur after a meal. In addition to a post-meal walk, set a reminder for a quick stretch or exercise after every 30 minutes of sitting to help maintain your glucose levels.

While incorporating frequent movement breaks into your daily routine can contribute to better metabolism, nothing replaces regular exercise incorporating strength training, a balanced diet with good protein intake, and medication in diabetes management. As always, speak to your clinician to tailor a health strategy just for you.

The peptide conversation is everywhere—and increasingly, women in menopause are turning to experimental peptides to address symptoms like weight gain and hot flashes.

As an endocrinologist, I understand why women are searching for solutions. Menopausal symptoms are real, and they need appropriate medical attention.

But many peptides being sold online are not FDA-approved for human use. Questions about product purity, dosing, safety, and long-term effects remain.

Before trying an experimental peptide, talk with a qualified healthcare professional about evidence-based options for your symptoms.

Thanks so much to @beardkenzie @nypost @nyulangone for having me.

Thanks so much to @insideedition for visiting my office and having me on to talk about the dangers of buying compounded retatrutide, or “reta,” from wellness pharmacies 💉

While retatrutide IS a real investigational medication with impressive clinical trial results, it is ultimately NOT yet FDA-approved. There’s an enormous difference between receiving an investigational drug in a controlled clinical trial versus buying it off the web/gray market or from a wellness clinic.

With an FDA-approved medication, the concentration, dose, and instructions are standardized and evaluated. When someone gives you an experimental medication claiming it is “reta,” you don’t know how much is in the vial, or even if it is pure or sterile. You are injecting something into your body without this knowledge, which is dangerous and can even be life-threatening.

I cover all of this and more with Inside Edition—stay tuned for the segment, coming soon 🔜

I’m often surprised by how many of my patients are not getting enough daily calcium intake, have vitamin D deficiency, and how many overcorrect without realizing it through supplement ‘stacking.’

As an endocrinologist at @nyulangone, here are my tips for making sure you’re getting the proper amount of vitamins and nutrients per day:

- Women under 50 should aim for 1000 mg of calcium, and women of menopausal age should aim for 1200 mg of calcium per day (this includes diet and supplement combined).

- Get your blood work done and consult your doctor for a personalized treatment plan. Inadequate calcium intake often will not show up in blood work, so getting additional guidance from your doctor is crucial.

-Remember: diet is always best! Make sure to include calcium-rich foods such as almond milk, yogurt, tofu, etc., in your diet.

- Examine your supplements carefully. Vitamin D is often incorporated into multivitamins, complexes, and sleep agents; taking overlapping supplements may lead to toxic levels of vitamin D.

Thyroid and pregnancy, what you need to know from endocrinologists and the new American Thyroid Association evidence-based guidelines

Pregnancy naturally changes how the thyroid works. A pregnancy hormone called hCG gives the thyroid gland a gentle boost, especially in the first trimester, which is a normal and healthy part of pregnancy, not a sign of thyroid disease. Because of this, TSH (the main blood test used to check thyroid function despite what quacks thinks) is often a little lower in early pregnancy than it would be outside of pregnancy, and it’s common and normal for TSH to dip below what’s considered “low” in a non-pregnant person. As pregnancy progresses, TSH gradually moves back toward typical non-pregnant levels.

What counts as a “normal” TSH in pregnancy

Because normal ranges shift during pregnancy, doctors ideally use TSH reference ranges specific to pregnancy trimester and to the lab performing the test. When those specific ranges aren’t available, a reasonable general guide for the first trimester is a TSH between about 0.1 and 4.0 mU/L, gradually shifting back toward standard non-pregnant ranges later in pregnancy. It’s important to know that these are general guides and your own doctor will interpret your results in the context of your health history and symptoms.

Goals for women with established hypothyroidism

If you already have hypothyroidism and take levothyroxine, the goal before and during pregnancy is to keep your TSH within the normal range, ideally under about 2.5 mU/L, to build in a safety margin, since pregnancy increases the body’s demand for thyroid hormone. Many women need a dose increase once pregnancy is confirmed, often around 25–50% higher by mid-pregnancy, with follow-up testing to fine-tune the dose. Women who stay in good control on treatment throughout pregnancy have outcomes similar to women without thyroid disease at all so the key is regular monitoring and timely dose adjustments with your care team.

Normal calcium levels in blood work can often mislead patients into believing their daily calcium intake is sufficient—in fact, many of my own female patients are surprised to find that they’re not getting enough calcium to maintain bone health 🦴

Not only do women tend to consume less calcium on average than men, women also have higher calcium requirements than men when approaching menopause—as a result of decreasing estrogen levels and higher bone loss during this transition.

I sat down with @verywellhealth to discuss why not getting enough calcium is more common in women, and strategies for women to get the recommended 1,000 and 1,200 milligrams of calcium in their daily diet.

As always, be sure to consult your physician before starting new supplements.

Peptides are everywhere right now. But what are they, and are they actually safe?

They recently made headlines after the Food & Drug Administration (FDA) met to discuss expanding access to certain peptide products, leaving many people wondering what that means for them.

Endocrinologist Dr. Rachel Pessah-Pollack is here to clear things up. She explains what peptides are, what the FDA’s recent actions do—and don’t—mean, and what you should know before considering one.

The recent FDA panel reviewing peptides on Friday has been an extremely divisive topic; I spoke with @nbcnews last Thursday to talk about why many clinicians are hesitant about prematurely approving these peptides.

The Pharmacy Compounding Advisory Committee has now recommended six peptides to be added to a drug compounding list, many of which have not yet been studied in humans and are sold on the gray market with no quality control for contaminants.

Thank you to Anne Thompson and @NYULangone for having me—this conversation is incredibly important to public health and consumer rights.
