I’ve noticed a shift in patient expectations over the years, particularly when it comes to how many concerns patients want addressed during a single visit.

And I think rising out-of-pocket healthcare costs may explain a lot of it.

When patients are paying hundreds of dollars for an appointment, [Myself including as I am a Patient] it’s understandable that they want to maximize that visit.

But as physicians, we also recognize that quality care requires time, attention, and sometimes multiple appointments.

I don’t think either side is necessarily wrong.

I think we’re seeing the downstream effects of how healthcare is financed.

Curious if other physicians are noticing this shift, too. And for patients, has the cost of appointments changed how you approach your doctor’s visits? by @drrawnbosley

Death, Taxes and Text Consults from Family and Friends…unavoidable things as a dermatologist.

But always happy to help. Just know @headandshoulders is recommended in my office AND for my family.

There have been some online chatter of people “annoyed” their derm recommended something over-the-counter but sometimes these options are legit, clinically tested and effective options.

Cosmetologists see the scalp up close every day. Observation is your superpower. 💛

Cosmetology school builds the foundation, but hair loss was not a priority in the classroom, and many cosmetologists learned hair outside of school. When the foundation is missing, misinformation spreads fast. So what is there to believe?

S.T.R.A.N.D.™ is an education program that teaches cosmetologists directly from board-certified dermatologists. Topics include hair biology, scalp infections, scarring and non-scarring hair loss, medical treatment options, and how to work alongside a dermatologist. Cosmetologists who complete the workshop become S.T.R.A.N.D.™ Certified. It was founded by Senior Cosmetologist Susan L. Peterkin and Board Certified Dermatologist Chesahna Kindred, MD, MBA, FAAD.

Just this month:
🗓️ Oct 5: Instagram Live with Dr. Rawn Bosley, MD, a board-certified dermatologist
🗓️ Mid-month: Check-in with S.T.R.A.N.D.™ Certified professionals
🗓️ Oct 17-18: Two-day hair loss workshop in San Francisco, led by board-certified dermatologists
🗓️ End of the month: Webinar, After the Diagnosis
🗓️ Nov 1: Free pop-up salon in McDonough, GA, with free styling and scalp screenings

Derms… are we making acne treatment harder than it needs to be?

A few things We think we could do better:

1. Underutilizing compounded medications.
Sometimes the best regimen isn’t sitting in a single tube. Thoughtful compounding can simplify routines and target multiple concerns at once.

2. Treating the acne—but ignoring what it leaves behind.
Active breakouts, post-inflammatory hyperpigmentation, and acne scarring often need to be addressed as part of the same treatment strategy. This is very important in certain populations where cultural norms emphasize these post acne marks and acne scarring over the Acne itself (e.g. Black and Asian populations)

3. Not setting realistic expectations.
Acne treatment is rarely an overnight fix. Meaningful improvement often takes 12–16 weeks—and patients deserve to know that from Day 1.

Clear expectations + comprehensive treatment + simpler regimens can make a big difference.

What would you add to the list?

Your dark spots aren’t impossible to treat. You’re just missing these two key pieces of information.

Here’s what actually fades hyperpigmentation (and what wastes your money):

1	Tinted SPF every single day, rain or shine. No SPF = no progress, period.
2	Hyperpigmentation is not scarring; therefore, it is treatable.
3	Patience. Real fading takes 8-12 weeks, not 8-12 days.

Photoprotection and consistency matter way more than the price tag.

Follow for more real talk on clear skin, no fluff. And tune in for next week’s topic where we discuss the most overrated and underrated treatments for dark spots.
