If I could perform one procedure all day every day, it would be…a matrixectomy. A partial permanent removal of an ingrown toenail. But I know many people would rather not ever undergo a procedure no matter how gentle I perform them, so my next favorite offerings are nail braces that help straighten the curved nail.

He had already lost a toe. His surgeon wanted to prevent him from losing more.

A recurring diabetic foot ulcer. Limited progress with conservative treatment. And a healthcare system where even relatively straightforward procedures can require weeks of authorizations and coordination.

Dr. Tea Nguyen, a podiatric surgeon and private practice owner, decided to build a different model.

Through direct care, she created a way to provide ongoing preventive foot care, communicate directly with patients, and perform appropriate minimally invasive procedures in her office.

For this particular patient, an office-based procedure helped his wound make faster progress within two weeks.

Her experience raises a larger question: How much of what delays treatment is medically necessary, and how much comes from the way we’ve organized healthcare?

Foot things

Found this offloading insole that’s a bit different than the PegAssist I’m used to. These are intended to take pressure off the bottom of the foot, for when we’re trying to heal a foot hole (ulcer) or in this case protect a foot filler injection site.

Curious to see how it holds up.
