Beyond Burnout: Designing a Sustainable EM Career

Preparing for this topic became unexpectedly personal. A subject close to my heart and something I’ve quietly struggled with over the past year.

Somewhere along the way, I realised burnout is not simply about workload. It is also about slowly losing connection to the purpose behind what we pursue.

It was an honour to present and moderate conversations on sustainability, burnout and the future of emergency medicine alongside inspiring colleagues from around the world.

This experience reminded me that perhaps the future of medicine is not about surviving alone, but learning how to sustain one another through growth, knowledge sharing and meaningful human connection.

Follow on ICEM2026 and BurnoutTalk stories for more updates.

Key lecture insights shared in the previous post!

What sustains you most in medicine?

Burnout in emergency medicine is NOT A PERSONAL FAILURE.

It’s a DESIGN FAILURE predictable to a system that was never designed to be sustainable.

Across Malaysia and globally, the pattern is consistent:
🧱 workload
🧠 cognitive overload
🏥 system pressure
⚖️ moral distress

And the cost doesn’t remain personal but manifests everywhere.

In our teams, decision makings and ultimately our patients.

So the reality is not just about “how do we cope?”

How do we design something better?

Next shift — survive safely.
Next month — recover intentionally.
Next year — redesign your career.

Because a sustainable career is not found.
It’s built and designed.

Here are key takeaways of my lecture at the 13th International Critical Care and Emergency Medicine Congress, Antalya Turkey. How are you feeling lately?

Let’s learn more together!

Pernah rasa nak pitam/ blur / confuse masa panas? 🥵

🚨 Most patients don’t suddenly collapse.
They show warning signs hours before.

But we miss them.

Because we:
Record vital BUT don’t interpreT
See abnormalitieS BUT don’t escalate

This is where NEWS2 matters.

🧠 A simple score.
📊 Built from basic vitals.
🚑 Designed to trigger action before it’s too late

🔥 Quick takeaways:
	•	Score 0–4 → Monitor

🔍 Topics Covered:
	•	Applied Airway Anatomy & Physiology
	•	Basic Airway Devices & Techniques
	•	The Difficult Airway Algorithm
	•	Pearls & Pitfalls in RSI
	•	Introduction to Basic Ventilator Settings

🛠 Hands-On Skill Stations:
 - Airway Adjuncts & Device Application
 - RSI Medications & Resus
 - Emergency Surgical Airway
 - Bronchoscopy in the Emergency Department
 - Case scenarios: Setting up VENTS

What truly stood out was the enthusiasm, curiosity, and active engagement from the participants that made our day!

A heartfelt thank you to the incredible organizing committee for the hardwork and dedication.

Special thanks to Suria Medik for supporting us with airway devices and video laryngoscopy/bronchoscopy to experience and play with!

May the knowledge gained today be applied meaningfully in each of their institutions and avoiding panic when managing airway emergencies 😬

A Letter for The Tired Heart.

Do it.
For the patient who needs saving.
For the team that looks up to you for answers.
For the student who learns by watching you.
For the family with hopes of a mountain.
For the legacy that lives on with every life you’ve touched
For the minds and future we shape.

Do it for Allah
Not for praise.
Not for glory.
But because every life saved is a chance to serve Him.

PRESSURE vs PERFORMANCE:
Cognitive Load & Emotional Intelligence on Clinical Outcomes

-Emergency Physicians face the HIGHEST burnout rates among all specialties.
-Over 60% experience EMOTIONAL EXHAUSTION and DEPERSONALISATION.
-Optimal cognitive load sharpens focus — but when OVERWHELMED, may lead to MEDICAL ERROR.
-Emotional Intelligence (EQ) isn’t just a soft skill - it’s a CLINICAL SUPERPOWER, bridging performance under pressure.

Sharing key insights from my presentation at the 12th International Critical Care & Emergency Medicine Congress & World Academic Council of Emergency Medicine, Antalya, Turkey (April 17–20, 2025) #emotionalintelligence #cognitiveload #conference #emergencymedicine #emergency #medicine #emergency taken in Starlight Hotel Antalya by @embitez

10 pearls in managing THE CRASHING PATIENT

1- SCREEN THE VITALS!
Be aware of subtle signs of shock eg- narrowing pulse pressure, shock index, relative hypotension in hypertensive patients

2- ANTICIPATE AIRWAY CHALLENGES EARLY
Always analyse patient’s anatomical and physiological challenges thoroughly.

3-ADRESS HYPOXIA AND HYPOTENSION
These two factors are major contributors to arrest patient. 2/3rd

4-HIGHER SBP PRIOR INTUBATION
-Higher BP with fluids should the patient be dry
-Usage of push pressors ; adrenaline or phenylephrine

5-LOW ANESTHETICS AND HIGH PARALYTICS
-Shock is a state of anesthetics. Patients do not require high dose of sedative agents. Prevent exaggerated sympatholytics effect of pre-meds.

6-VENTILATOR STRATEGY
-Titrate PEEP and Tidal Volume accordingly (4-6mls/kg)
-Too high PEEP and Tidal Volume may impair venous return and cause shock

7-VISUALISE YOUR STEPS AND STRATEGY
-Failing to plan = planning to fail
-Risk stratify the patients, address the challenges and prepare yourself, team and equipment

8-DONT MISS LIFE THREATENING CONDITIONS IN ED!
-3 most lethal diagnosis that may arrest patients:
Tension pneumothorax, cardiac tamponade and massive PE

9- USE THE ULTRASOUND
-POCUS - RUSH protocol in shock patients

10- SHARE THE BURDEN ADEQUATELY
-Do not do things in silo
-Involve your superiors and critical care team early by @embitez

EMBitez3: Hot and Altered!
Serotonin Syndrome (SS) VS Neuroleptic Malignant Syndrome (NMS)

1-Diagnosis of suspicion that are often missed!
2-Thorough past medical and drug history is important eg- antidepressants (SS) and anti-psychotics (NMS)
3-Overactivity of serotonergic activity in SS VS dopamine cessation and sympathetic Overactivity in NMS
4-SS myoclonus is a distinguished feature whilst NMS patients are often rigid!
5-Treatment are mostly symptomatic with IV hydration, external cooling and benzodiazepines
6-Specific treatments include cyproheptadine (SS) and Dantrolene (NMS)

Enjoy this EM pocket graphic and have a great weekend!
