How Chronic Stress Accelerates Aging and Harms Metabolism

Dr. Datta discusses how chronic stress elevates cortisol, which accelerates aging by increasing inflammation, promoting fat storage, suppressing thyroid and testosterone, and disrupting sleep and memory. She emphasizes that managing cortisol is key to recovering metabolism, and highlight social connection as a crucial factor for metabolic and hormone balance.

When I started my own GYN boutique office in 2017 I never anticipated prescribing hormone therapy, but over and over again patients showed up struggling with low energy, brain fog, poor sleep, weight gain and honestly loss of quality of life and their identity. I realized I could not help them with my limited knowledge on menopausal management despite having already been in practice 10 years! I needed to level up so I dove into hormone therapy and realized I knew nothing about testosterone - the data was thin so I went searching.

Courses. Conferences. Late nights on the phone with clinicians who’d been doing this longer than me. Partnerships with clinical pharmacists on methods of delivery, dosing & titration. All while newly married with a new blended family of a teen, a tween and a newborn - and it was absolutely worth it.

Small patient numbers. Controlled. One variable tested at a time. Low and slow dosing, adjusted in increments most training never covers.

Every visit, a candid conversation about where the data ends and clinical judgment begins.

Then my menu expanded. New combinations with estrogen and progesterone. New patient profiles. New questions.

Ten years of watching what testosterone actually does in the body once you’re paying close attention. Libido is the approved indication, but it’s not the whole story - I can’t unsee it now. Cognition. Metabolic health. Cardiovascular health. Body composition. Tissue repair. Intimacy. Mood stability. Agency. Confidence.

The research & conversation is still catching up in a bigger way to what clinicians doing this work early have been observing in the room.

Do individual results vary? Yes. Is hormone therapy a MUST? No. Does it replace essential pillars of health like diet, movement, sleep and stress optimization? No.
It is an important tool we’ve likely under utilized and medical professionals and patients deserve to understand it fully.
Keep. Being. Curious. Keep. Learning.

The Hidden Impact of Chronic Stress on Evening Cortisol and Women’s Health

Dr. Shoma Datta discusses a powerful research example by Cicciolo and Lizar, showing how chronic psychological stress, particularly the invisible mental load of managing a household, keeps evening cortisol elevated when it should drop. This disrupts sleep architecture and suppresses reproductive hormones, leading to clinical symptoms like unexplained weight gain, fatigue, and brain fog in women in their forties.

RELEASED TODAY 8/18/26: International Federation of Gynecology and Obstetrics (FIGO) and the International Menopause Society (IMS) released a joint position paper calling for a fundamental shift in how hormone therapy (HT) is approached for premature ovarian insufficiency (POI) and early menopause (EM). The proposal moves past treating these as variants of typical menopause and instead frames them as chronic endocrine conditions — sentinel events for accelerated aging — that call for physiologic hormone replacement, individualized duration, and integration into long-term preventive care rather than symptom-driven, time-limited treatment..

Starting a series breaking down the paper’s six key points — this video covers the overview:

1️⃣ Impact beyond reproduction — cardiovascular disease (especially ischemic heart disease and stroke) accounts for much of the excess mortality in POI/EM; accelerated bone loss raises fracture risk; both conditions are linked to greater frailty and measurably lower quality of life across physical, emotional, and sexual domains
2️⃣ Why prompt diagnosis matters — delayed recognition prolongs untreated hypoestrogenism
3️⃣ HT dosing — typically DOUBLE standard postmenopausal doses, targeting physiologic estradiol replacement rather than symptom-based minimal dosing; strongest evidence for this higher-dose approach comes from bone outcomes, where conventional doses have been shown inadequate to prevent POI-associated bone loss
4️⃣ Management when systemic HT is contraindicated
5️⃣ Education and research priorities to close current gaps
6️⃣ The new paradigm for healthy aging — a true life-course model

Follow along as the series continues.

How Walking and Strength Training Boost Insulin Sensitivity and Testosterone

Dr. Datta and Dr. Thomas discuss how regular walking improves insulin sensitivity by helping muscles absorb glucose without relying heavily on insulin, reducing fat buildup and inflammation. We also explore the critical link between insulin resistance and testosterone decline, emphasizing that strength training further supports natural testosterone and growth hormone levels, making these exercises non-negotiable for men and women after forty.

MY GREATEST LONGEVITY SECRET WEAPON: MY MARRIAGE

Happy 10th “Caliversary” to my biggest fan in public, coach in private, and unfair advantage @drjohnthomas . There was a time I thought unrelenting independence was the only way, but I realized true partnership can be a force multiplier. Thank you for a decade of deep trust & laughter - showing me that a strong marriage is a foundation for longevity, peace, and growth. For me, everything starts with us and it all builds from there. 10 years in and a lifetime to go 🥂 🩵

Life looks different for everyone but IF you choose to marry, choose wisely and keep working at it - data consistently shows a healthy partnership acts as a buffer against chronic disease and significantly boosts career trajectory.

📊 Supporting Studies:

🧬 The Longevity Factor (Psychological Bulletin): A massive analysis of 72,000 individuals confirmed that high-quality relationships heavily lower mortality rates and protect your heart.

🧠 The Lifelong Predictor (Harvard Adult Development Study): This multi-decade study proved that supportive relationships are the single strongest predictor of physical longevity and sharp brain health in old age.

💼 The Career Boost (Psychological Science): A 5-year study of 4,500 people proved that a partner’s conscientiousness directly predicts your future promotions, job satisfaction, and higher income. by @everydaylongevityco
