When we think about osteoporosis and fracture prevention, we tend to focus on bone.

But osteoporotic fractures usually occur when someone falls, not just because of weak or fragile bones.

That means fracture risk depends on both bone strength and the likelihood of falling.

This is where strength training becomes so important.

Strength training can help improve or maintain bone density while also improving the very factors that reduce the risk of falls, including muscle strength, muscle power, and balance.

Someone with better balance is less likely to lose their balance, and greater muscle strength and power make it easier for them to react quickly and 'catch' themselves before a fall occurs. 

If a fall does happen, stronger bones are better able to withstand the impact, reducing the likelihood of fracture.

This is where effective osteoporosis management involves building a stronger musculoskeletal system as a whole, not just protecting bone.

That’s why consensus recommendations for osteoporosis increasingly emphasise resistance and balance training as core components of osteoporosis management, alongside appropriate medical care where indicated.

Preventing fractures is not just about bone, because healthy bones rely on healthy muscles.

And strength training is one of the few interventions that can simultaneously improve bone health, muscle function, and balance, helping people stay active, independent, and confident as they age.

This Dementia Action Week, we're recognising Henry Graven, Principal Physiotherapist at Kieser Sandringham.

Earlier this year, Henry challenged himself to complete the 2.5km Great Bayside Swim to raise funds for @dementia_australia, in honour of his grandmother, who sadly passed away just one week before the event.

With support from his sponsors and the Kieser Bayside clinics, he raised $3,260 to support people impacted by dementia, and finished the swim in just under 49 minutes.

Today is World Clinical Exercise Physiology Day, recognising Accredited Exercise Physiologists and the role they play in helping people manage their health through exercise.

Many people are surprised to learn that exercise physiologists help people to manage a wide range of health conditions, including type 2 diabetes, heart disease, osteoporosis, stroke recovery, arthritis, cancer, neurological conditions and persistent pain.

They take the time to understand how a condition affects each individual, then develop a personalised exercise plan based on their health, goals and current capacity. This may include health checks, strength and movement assessments, supervised exercise and regular reviews to track progress.

This week is Dementia Action Week - an important reminder to think about what we can do to support our brain health as we get older.

And while we often talk about the benefits of exercise for brain health, we tend to think of strength training more for what it does to our muscles and bones.

But what actually happens to our brain when we start lifting weights?

Researchers are starting to look beyond whether strength training can improve thinking, learning, and memory - to what might actually be changing inside the brain itself.

And a 2025 study (by Kušleikienė and colleagues) provides an interesting example.

Researchers had adults aged 60 to 85 complete 12 weeks of lower-body resistance training, while using MRI scans to examine changes in the structure of their brains.

Among participants at greater risk of mild cognitive impairment, strength training was associated with positive changes in brain regions involved in learning and memory. Some of these changes were linked to improvements in cognitive performance.

Of course, one relatively small study can’t tell us that strength training can prevent dementia.

But it adds to a broader body of research suggesting that the benefits of strength training may extend well beyond our muscles.

Recent reviews of multiple studies have found that resistance training can improve aspects of thinking, learning, and memory in older adults.

Exactly how strength training affects the brain is still being understood. But the bigger picture is becoming increasingly clear: when we strength train, our muscles aren't the only part of the body that can adapt.

So during Dementia Action Week, it’s a timely reminder that keeping our bodies strong as we get older may also be one way of supporting the health of our ageing brains. by @kieseraustralia
