Laser tattoo removal with titanium dioxide (TiO₂), white ink, and flesh-colored ink requires an abundance of caution.

Titanium Dioxide (TiO₂) & White Ink:
These pigments are highly reflective and don’t absorb laser energy well. When exposed to laser light—especially pigment lasers—they can oxidize and darken, turning gray or black. This is called paradoxical darkening. The treated area may end up looking worse, and in some cases, the darkening can be permanent.

Flesh-Colored Inks:
These often contain TiO₂ or iron oxides. Iron oxide can oxidize into ferric oxide (a rust-like color) when treated with a laser, leading to a reddish or darkened appearance. Because these pigments are designed to match skin tone, any darkening is especially visible.

This is a good reason to wash an area thoroughly to remove makeup and sunscreen prior to treatment. It is also a good example of why it is imperative to choose a well-trained, experienced laser surgeon for your treatments.

The FDA has approved updated labeling for Kybella (deoxycholic acid injection), adding warnings about serious adverse events associated with injections outside the approved submental region and persistent nodules or masses at injection sites.

In its September 15, 2026 safety communication, the FDA emphasizes that Kybella is approved to improve moderate to severe fullness associated with fat beneath the chin in adults. Treatment of fat elsewhere on the body is not approved and is not recommended by the agency.

The FDA identified 129 reports of adverse events involving injections outside the submental region. Reported complications included blurred or reduced vision with periorbital injections, facial weakness, and muscle or nerve injury.

The agency also identified 117 cases of unresolved injection site nodules or masses. At reporting, these had persisted for an average of 143 days. Such reactions can occur following treatment within or outside the approved region, after initial or subsequent injections, and may require medical intervention.

The FDA advises clinicians to review prescribing information and counsel patients about adverse reactions. Patients should seek medical attention for complications, including an asymmetric smile, facial weakness, difficulty swallowing, or serious injection site reactions. Suspected adverse events should be reported through the FDA’s MedWatch program.

Here is the FDA guidance for more information:

The most important decision you will make in achieving a safe, successful cosmetic outcome is who performs your treatment.

Our published research in 2013, 2014, and 2023 consistently demonstrated that the safety of cosmetic procedures is closely linked to the training, experience, and expertise of the person performing the treatment#dermatologistasbd #boardcertifieddermatologist #cosmeticdermatology #dermatologo #ownyourexpertise .

Before undergoing any cosmetic procedure, don’t just ask about the technology or the product—ask about the person performing it. How were they trained? How much experience do they have? Do they perform these procedures every day? taken in Massachusetts General Hospital by @mathew_avrammd

While fractional lasers are well known for stimulating collagen production, that’s really just one part of the story. Increased collagen is actually a reflection of the many healthy changes these treatments create in the skin.

Fractional lasers help replace older, damaged, senescent cells with newer, healthier cells. The result is skin that functions better, protects itself from sun damage more effectively, and has improved texture, tone, and pigmentation—all while producing more collagen.

Even more exciting, research from our team has shown that fractional laser treatments can reduce the risk of common skin cancers by approximately 50%.

By improving skin health at the cellular level, enhancing collagen production, and helping protect against future sun damage and skin cancer, fractional lasers are among the most effective treatments available for both the health and beauty of your skin.

What a wonderful year with @bridgetmyersmd and @drchouderm at the Harvard Laser & Cosmetic fellowship. They have seen and treated thousands of patients in their laser and cosmetic fellowship since last July providing outstanding care. At the same time, they have been in the lab creating a new concept in skin care that you may be using someday. Additionally, they have published several peer-reviewed papers, delivered several abstracts, and spoken at multiple national meetings - all the while keeping their friendly energetic demeanors. Most importantly, they are wonderful people. They are close friends with each other and touched all of us over the past year. We are sad to see the fellowship year come to an end, but we are so excited to see all that you will accomplish in the years ahead.

Did you know the strength of the sun is driven more by the calendar than the temperature? The most intense UV exposure occurs around June 21 (summer solstice), while the weakest occurs around December 21 (winter solstice). Peak UV hours are typically between 11 a.m. and 2:30 p.m., when the sun is highest in the sky.

This means your skin is at greatest risk during summer months and especially during midday hours—even on cooler days. This is also true on cloudy day, when UV rays still can harm the skin. To protect yourself, seek shade, wear wide-brimmed hats and protective clothing, and apply a broad-spectrum sunscreen regularly.

A little awareness goes a long way in reducing sun damage and lowering skin cancer risk. Stay safe and enjoy the summer.

Before-and-after results of my CO₂ ablative fractional resurfacing by my fellow @drchouderm. The baseline image is from August 2025, with the post-treatment result in May 2026. This technology creates thousands of microscopic channels in the skin, stimulating cellular turnover, increasing collagen production, and promoting healthier, more resilient skin. In addition to improving tone and texture, it smooths wrinkles and enhances the skin’s defense against UV damage. Thus, it provides a natural cosmetic benefit while also improving the health & function of the skin.

The accompanying images illustrate my one-week recovery process, from immediate post-treatment to one week post-treatment.

This is a powerful, aggressive procedure that delivers meaningful results—but it requires significant expertise. Treatment should always be performed by a highly-trained, experienced expert.

We are seeing increasing reports of counterfeit botulinum toxin products, including Botox, being used in some medi-spas across the country. Most recently, the U.S. Food and Drug Administration issued a warning to a medi-spa in Texas to stop using counterfeit products. Unfortunately, this is not a new issue—we have also seen concerning reports in the Boston area.

In rare cases, improperly sourced or poorly administered injections have led to symptoms consistent with botulism - something that should never occur with legitimate care. When performed correctly, botulinum toxin treatments are extremely safe with a long track record of minimal serious side effects.

The key takeaway is simple: carefully vet both your provider and the clinic. Choose experienced, reputable medical professionals—because in aesthetics, safety depends on who is holding the syringe.
