For background, miss girl had a R CVA last June resulting in L hemiparesis at her shoulder, elbow and hand. She does wonderful at incorporating her L side as much as she can but her lack of extension makes grasp/release/reaching/transporting very difficult.

• Coban wrap can provide sensory input, reduce swelling, reduce joint stiffness, and improve dexterity (specifically targeting extension).
• Although not recorded, before the coban was applied, pt couldn’t grasp these cones or release them if they were placed in her hand by me.
• Lastly, reaching to floor allows gravity to assist with promoting elbow extension as pt stands upright with elbow at 45* flexion naturally. With the coban, pts LUE elbow extension is almost -10*.

We love a good win! Side note, she was able to pick up her cats toy and toss it for him after this 🥹 (yes, he plays fetch) by @otwithlexi

One of my favorites for toileting management! Keyword, ONE of my favorites! No patient likes to practice toileting live — specifically when dignity, pride, and self image are being reserved as patients age. Here’s a fun way to keep things simulated, while keeping your patients confidence high! by @otwithlexi

FLOOR TRANSFERS ✨
Why do we practice? In short - to decrease fear and poor reaction following a fall, utilize problem solving to get to a safe surface, check vitals to avoid syncope or BP drops/increases etc. Not only does this facilitate safety for fall prevention but also helpful for the population with grandchildren or pets when frequently transitioning from standing to floor level

Step 1: ensure hips are properly stretched and warmed up before jumping straight to floor level tasks
Step 2: provide BUE support on chair or sturdy surface nearby during tall kneel <> lunge transfers requiring hip mobility and strength, feet clearance on floor without dragging foot, and proper foot placement in front of trunk
Step 3: progress from the lunge to stand with continued BUE support, ensuring slow pace to avoid dizziness from frequent changes in position

🐦🌳🦜🦉🐤This all started from sitting with the sliding door open during the session, reminding my sweet patient it was time to fill the bird feeders!
When I asked what it involves, I was not prepared for his answer! He began to tell me that the 10-12 lb bag was in his garage (involving 1 step down into garage and more items)… all in the front of the house when the rest of the task was in the back yard. So what did we do?
• put the heavy food bag in a reusable bag with handles
• take breaks while walking through the house due to imbalance of weight (suggested alternating hands) while using cane in opposite
• put down bag while opening any doors/negotiating thresholds
• store bag outdoors near bird feeders for future use to avoid reoccurring heavy lifting (he refills them 2x/week)
• use the patio table as a working station to reduce need to stand for duration in grass/uneven terrain (in heat especially)

What do we still need to figure out?
• how to safely transport filled bird feeders from patio table, then reach + place them back on overhead hook due to shoulder pain and uneven surface to step on (flower bed/rocks etc)??? any ideas?

My girly has had a R ankle anatomical deformity after a break several years prior. Due to this, her hip and knee tend to compensate causing poor sit to stand body mechanics and posture. To assist in muscle memory and using other musculature, pt provided with tactile cue between legs! by @otwithlexi

⚠️⚠️ TW: ACUPUNCTURE NEEDLES ⚠️⚠️
I’m sharing to help people understand a little more about the conservative treatments for muscular pain, spasms, reduce joint stiffness for arthritis and decrease tension headaches.
Transparency is important. Initially, as he places the needles, you feel a small pinch for a nano second, literally. Certain areas you feel more than others, like the lower back. The only thing you feel is the initial placement. While I lay face down for 30 mins, I feel nothing. If you contract, activate or flex a muscle that the needle is in, you’ll notice it’s there but that’s it. Its job is to relax your muscles, so as you lay you feel yourself sink into the table. You want to avoid distractions from your phone, watch, etc to immerse yourself. My office has meditative music playing in the background.

The reason the pictures have a red hue is because I also receive Far-Infrared Heat from a lamp to warm and heat deep tissue directly (which increase healing).

I specifically have a confirmed C3-C4, C4-C5, C5-C6 bulging disc and 2 annular tears (around the edge of the disc). I also have had scoliosis since 8th grade (2012-2013) which stopped at a 29* curve but affects my cervical, thoracic, and lumbar spine which means the likelihood of more bulging discs or tears along the length of my spine is high. The right side of my back is elevated when I lean forward due to my spines rotation as well. All of this leading to muscular imbalance and constant adjustment to support my bones. This chronic (constant long term) adjustment leads to muscle spasms over time.
With a thorough understanding of my body, anatomy, physiology and function, I’m able to work together with my acupuncturist to target the muscles working the hardest with most tension through palpitation and locating high pain areas. (I could probably have a needle covering every square inch of my back and neck 😌).
I done a total of 7 sessions so far, 2x per week the first two weeks, I skipped one week, 1x last week, and 2x this week … for a combined 5 weeks. I stretch in the morning and before bed, use a foam roller, and use heat packs. So far, a win! by @otwithlexi

My guy here has taken a few falls due to gradual decline in size of movements. Posture was rounded, forward, hunched over with very small unsteady steps even while using a device.

Luckily, with a little modification to the environment, LOTS of stretching in seated, and some added cues for emphasis… we’ve got some pretty solid walking going on! This patient specifically likes to walk down the hall (about 100 ft) to play cards with friends, and he wasn’t able to make it there (safely at least).

After the past two weeks of a lot of work involving what is shown, my guy can stay to the right of the hallway with improved stepping and pacing.

Shoutout to @foxrehab for allowing their clinicians to make their own schedule, allowing me to grab coffee when I’m rundown, check in at my old place of work to visit old clients and help another OT and move patients to another day if something comes up! A good pace throughout the work day really helps to avoid burn out. I love what I do 🤍🤗 by @otwithlexi

I love finding creative items that I have lying around to use for activities (minus theraputty). Some of the items are the patients! The best part about this one is the patient was smiling and laughing of the whole time, even if items were dropped. This patient has carpal tunnel, since sx, now has neuropathy in all fingers, affecting grasp, strength and sensation of different items to determine what they are or how to hold them correctly.

1. Threading beads on pipe-cleaner
2. Assembling and disassembling small Lego tower
3. Stereognosis/sensation task
4. Clothespins tweezer transport task
(Not recorded)
5. Removing 8 beads from medium theraputty
6. Wringing putty for wrist rotation
7. Putty pinches (pincer and lateral)
8. Opening and closing each container (snap lid vs rotation)

Why? Patient has difficulty retrieving medication from organizer to place into opposite palm to take, difficulty removing/fastening clasp of jewelry or donning hearing aids, difficulty determining the item in hand when rummaging through her purse, and lastly performing laundry tasks to hang or remove clothes from clips!

How to upgrade/downgrade: using a string for more difficulty or a wooden stick to decrease challenge, use larger legos to increase ease or have pt pull legos out of bag to assemble by size, add rubber bands to bottom of clothespin to add resistance, or use smaller clothespins to decrease challenge. by @otwithlexi
