Wow, this somehow snuck past me last month...we have officially crossed 1 million views on YouTube!

When I started making videos, I really just wanted to make sleep medicine education more accessible and practical for my colleagues and patients.

Not to mention, I was terrified of being on camera!

It’s pretty wild to know those videos have now been watched more than a million times.

Thank you to everyone who has watched, shared, subscribed, or sent a video to a colleague.

Here’s to the next million!

It was my first time attending the @californiasleepsociety conference but certainly not the last!

Congratulations to the organizers for a fantastic event, and a huge thanks to my friends and colleagues Dr. Rafael Sepulveda @sleepfitmedical and Dr. Sam Kashani @samkashanimd for the invitation to speak on behavioral treatment of insomnia.

I learned so much from all of the speakers, attendees, and exhibitors. In this digital world, there’s no replacement for analog, authentic connection.

Looking forward to the next one!

I had so much fun meeting @sleepdrpm Dr. Pegah Mashayekhi DO, and appreciate her for sharing her wisdom at this year’s @californiasleepsociety conference in Sonoma!

If you’re a PCP or psychiatrist whose patients are afraid of getting a sleep study because “I don’t want to wear that mask” - remind them that there are lots of options and sleep specialists will help them personalize the treatment plan.

I loved this sleep quote from Theo of Golden by Allen Levi:

“And he was pleased, in these unsought moments of sleeplessness, to sit and surrender to the silence, his mind free to take him wherever it wished. He found in such moments the chance to draw upon memory and recall scenes from his long, storied life.”

What are you reading right now?

A trap that clinicians often fall into unwittingly is this:

Feeding into your patient’s anxiety.

This is especially true when it comes to insomnia.

When your patient can’t sleep, they’re desperate for relief and are coming to you for help.

So you reach for the tools in your toolbox: sleep hygiene, prescriptions, magnesium, CBT-i, relaxation practices, and so on.

The patient gives it a go, and then comes back still struggling.

So you adjust the plan. You try something else, search OpenEvidence, or maybe ask your colleagues in a Facebook group for recommendations.

Then the patient tries that and it’s still not helping.

And now you’re in what I call the “Dance of Sleep Effort”.

The patient is trying really hard to overcome insomnia, and you’re trying really hard to help them (and maybe even getting frustrated yourself).

All the while, the effort you’re putting into the plan is actually fueling the patient’s anxiety, desperation, and fear that nothing will “work”.

I see this pattern come up all the time when I teach clinicians about sleep.

The paradox of insomnia is that it actually needs less effort, not more - both on the part of the patient and clinician.

❗️Hot take:  Only psychiatrists and psychologists should be allowed to do CBT-I, cognitive behavioral therapy for insomnia.

I heard this from a sleep medicine physician colleague.

They told me that some people think sleep medicine doctors, like pulmonologists, neurologists, and non-psychiatry trained sleep medicine doctors should not be doing CBT-I because this is in the domain of therapy.

Those of us in psychiatry know there are nuances to therapy that most docs aren’t taught.

Thus, some think that only psychiatrists and psychologists should provide CBT-i.

Kind of a hot take.

I have my own thoughts on this which I’ll share later.

❓ What do you think? Should psychologists and psychiatrists be the only ones allowed to do CBT-I? 👇
