I have a bias when I am managing a wear case.. i do not want to prep the already damaged teeth…. I don’t mean crown preparations… I mean zero prep (other than alumina) Orthodontics, opening the OVD, choosing between CR/MIP, the Dahl concept are my big guns to be fully additive Composites (not just injectables) require regular maintenance and in this case some wear and staining are evident…. but the patient described it well: ‘KK, I am so grateful you did not cut my teeth down’. A 5 year follow up of an additive, interceptive management of the worn dentition using monoshade injectable resins. The wear was primarily intrinsic erosion with a secondary attrition element. Following orthodontic treatment, all lower teeth and upper anteriors were restored using the the injection moulding technique in 2020. Alumina air abrasion, a universal adhesive and a highly filled flowable were used. 4 small composite repairs were needed over the 5 year period and involved alumina air abrasion, silane application followed by the universal adhesive and flowable as per @marco.gresnigt repair protocol. Some key features of the favourable survival: - patient is fully aware of the consent process and maintenance needs - Excellent interdental cleaning and regular Hygiene visits - Nightime acrylic appliance - Annual polishing of the composite restorations using proximal strips and spirals. - Comprehensive occlusal control: recorded CR occlusal records at desired OVD, immediate posterior disclusion, shared edge to edge contacts and being in control of overbite and overjet. Lastly extensive test drives both on simulation software and intra orally through a mock up process. Additive, restorative Intervention with fully guided resins offers a conservative and cost effective rehabilitation and disrupts the trajectory of wear progression. The implication of zero intervention can sometimes be synonymous to supervised neglect. Ceramic restorations are therefore postponed thereby preserving tooth structure and reducing biologic and financial costs. When ceramics are deemed a suitable upgrade the existing bonded composites can act as a core by reactivating the composite surface and IDS if dentine is exposed. #additive #adhesive #interceptive #Cost effective #aesthetic #repairable #injectable by @prosthoworks
