💥SMILE MAKEOVER - TOOTHWEAR💥

This patient came to us wanting a solution for his teeth wearing down and chipping away 👎

After a thorough assessment we found tooth surface loss (toothwear) to be affecting all of the front teeth. Meaning the teeth had become smaller but also we had lost space on the bite.

After discussing all the options we decided together to treat the toothwear with composite bonding and to restore a healthier bite. This gave us the space we needed to build the teeth up back to natural looking proportions.

After a bespoke digital design from @art_dental_laboratory , we were able to copy and paste the smile design straight into the patients mouth, using an injection moulding technique, for a flawless finish ✅

8 upper and lower composites set in CR at an increased OVD for this long lasting great result 🦷🔥

📉 BEWE, TWI, or Smith & Knight? The Tooth Wear Dilemma.

We are seeing more Tooth Surface Loss (TSL) than ever before, but are we all speaking the same language when we record it?

In our latest scoping review, just published in the Journal of the Irish Dental Association, we dove into the literature to answer a critical question: Is there actually a universal index for recording tooth wear?

The reality? It’s complicated. Without a standardized way to measure wear, monitoring progression and communicating with colleagues (and patients) becomes a major challenge.

With any composite restorations (and injectables) the staining usually occurs in areas where access is hard: PROXIMALY

Polishing spirals were typically rather thick and would not engage the embrasures. I needed an extra thin polishing spiral to achieve that! After a collaboration with @eve_rotary the new Diacomp Twist Plus XT delivers excellent adaptation and surface gloss.

It comes in a medium coarseness (pink) that will remove volume and a fine (grey) that will polish.

If these will not shine up your composites there is something wrong with your composite. They are that good!

- light pressure
- low revs at 3-5k rpm
- keep the surface wet
- constant movement

An ABC Smile Makeover for this superstar patient ✨

She wasn’t happy with the spacing in her smile or the overall symmetry.
We started with pre-restorative alignment to carefully redistribute the spaces, before finishing with 4 additive IM composite veneers to close the remaining gaps and refine proportions — all while keeping things natural and minimally invasive.

📸 After photos taken on the day of bonding, as she was flying back to the States the next day.
The gums will continue to settle, and we’ll be getting review photos at her next visit next year 👌

Full Arch Rehabilitation: The Power of Injection Moulding 🦷

The first image shows the transition from the initial presentation to the result directly after finishing. We are now allowing for a period of tissue healing before the final review to ensure optimal gingival health and integration.

This case presented a unique set of challenges that required meticulous planning and execution. We transitioned this patient from worn dentition to a full-contour restoration using the injection moulding technique. Through careful planning, we have increased the patient’s OVD in CR to provide the necessary restorative space and improve functional harmony.

The Challenges:

Upper Arch Management: Managing the entire arch for a harmonious smile.

Diastema Closure: Ensuring proper emergence profiles and contact points.

Full Contour Anatomy: Restoring lost vertical dimension while maintaining natural aesthetics.

While the lower arch restoration is planned for a later date once the patient is financially ready, we moved forward with the upper arch to protect his severely worn dentition. Although treating a single arch in isolation can be a compromise, this work provides the patient with immediate protection and service until the next phase of treatment.

The Process:
