The most beautiful thing about this transformation isn’t her new smile. It’s what that smile means to her. 🤍 Today is a time to reflect on what we’re thankful for and one of the things I’m most grateful for is the trust patients place in us to help them feel confident again. Would you glue something together in a hot shower? 🚿 Probably not, but that gives you a surprisingly good idea of the environment a dentist is working in when we bond a ceramic restoration inside the mouth. A study measuring household showers with water around 40°C found relative humidity averaging approximately 88–91% during showering (Kostecki et al., 2024). Now compare that to your mouth. Without proper isolation, the intraoral environment has been measured at >95% relative humidity (Kameyama et al., 2011). And with cotton rolls alone, 100% relative humidity was found in every subject studied (Haruyama et al., 2014). So, for me, sorry optragate and cotton rolls but buccal retraction is not the same thing as isolation. Moving the cheek and saliva out of the immediate way may improve access and visibility but when we’re adhesively delivering a handcrafted ceramic restoration, it is mandatory to control of the entire environment. That’s why you’re seeing all of this preparation: 🦷 Rubber dam 🧵 Floss ligatures 🔩 B4 clamps 💧 Complete moisture control 🤍 A customized, handcrafted ceramic crown being precisely delivered Studies show when a single tooth was properly isolated under rubber dam, relative humidity dropped to approximately 45–50%, essentially the same as the surrounding room at ~51% RH (Haruyama et al., 2014). Think about that difference! We’re essentially taking a hot-shower environment and turning it into a controlled room environment before bonding your restoration. Why go through all that effort? Because when someone invests in their smile, the goal isn’t simply to make it look beautiful on the day it’s inserted. The goal is to create the cleanest, most controlled and predictable environment possible so we can respect the adhesive protocol, the ceramic, the tooth and ultimately give that restoration the best opportunity to perform beautifully over time. A smile cannot be rebuilt with zirconia alone. First and foremost, it requires a healthy biological foundation. The patient presented with unaesthetic dental restorations, discomfort, and impaired masticatory function. Beneath the existing restorations, the teeth were severely compromised and could no longer be preserved. In the mandible, the implants showed significant bone loss and active infection, accompanied by extensive purulent discharge. The treatment had to address four essential objectives simultaneously: • eliminate the infection and restore tissue health; • rebuild a stable foundation; • establish a balanced occlusion; • achieve a natural, esthetic result. We digitally planned the entire case, beginning with the ideal position of the future restorations. Using guided surgery, we placed the implants in the planned positions. The rehabilitation was completed with fixed zirconia bridges in an All-on-6 configuration—FP2 iBar for the maxilla and FP3 for the mandible @restore.dentallab The outcome is about more than a new, bright smile. This is a full-mouth rehabilitation completed with the Star Concept protocol, our minimally invasive approach to implant dentistry, built around precise digital planning and flapless implant placement. In this case, the natural teeth could no longer be preserved due to systemic disease combined with advanced periodontal disease. A complete rehabilitation of both arches was planned, with 6 implants in the upper arch and 6 implants in the lower arch. All 12 implants were digitally planned before treatment and placed using surgical guides, without conventional open surgery. The final restoration was designed as a screw-retained FP1 prosthesis, without a prosthetic gingival component, preserving the patient’s natural gingival architecture.
