The gender gap in health care is very real…. The majority of medical research is undertaken on males, and information is simply assumed as relevant to female patients. Of course, this is not how health or disease work. 

Female hormones act as far more than just sex steroids, and that their significant systemic influence on health and disease has been largely overlooked. If we ignore the gap in the scientific literature.... we deliver a significant disservice to all of our AFAB patients. 

This masterclass will include everything I've always wanted to to share about sex hormone assessment & management for healthcare; beyond fertility, pregnancy and menopause. We'll explore how understanding the many roles androgens, oestrogens and progesterones play in female physiology is essential to wholistic care of every female patient. Because I'm utterly sick and tired of seeing women and AFABs treated as mini-men, and getting poor outcomes. And it happens ALL THE TIME.

This is where we must go beyond the limited evidence and current established scientific 'norms'. Where we lean into clinical specificity. Where experience matters. This is where we fuse the genuine established facts with our wealth of clinical data and nuanced insight to make sense of what's really happening for the people we care for. 

Because the very real existence of medical misogyny in research and practice has let us down, but we will not do the same to our patients.

New ASRM guidelines for RPL have just been published for the first time in 14 years. 

While there are some positive changes to these updates, we also think there is room for further nuanced evaluation and treatment in this patient group.

Since I started practising years ago, I noticed a pattern: when couples faced fertility challenges, it was almost always the female partner who was making appointments, doing blood tests and scans, and going through expensive, invasive procedures. Meanwhile, the male factor was often underestimated, under-investigated, and undertreated.

Even when there were clear male fertility issues, there were rarely guidelines or steps for males to actively improve their reproductive health. What solution were most couples offered? IVF. And that was it.

The truth is, the chances of success are significantly reduced if the male factor isn’t addressed. Ignoring male reproductive health places the weight of responsibility entirely on the female partner and often limits the couple’s chances of conceiving.

It's never too late. 💪

For decades, exercise science built its guidelines almost entirely on male bodies. This first of it's kind meta-analysis & systematic review (126 studies, 4,019 women) asked 2 questions:

💙 What is the effect of upper & lower limb resistance training in women/ AFABs?

💚 Does age, training frequency, or training volume change the effect?

The researchers concluded that:
✅ Strength gains happened in both pre- and postmenopausal women
✅ Lean & functional increased, while fat mass decreased across all ages
✅ Menopause didn't blunt the response

The researchers found 2 gaps when they could not definitively answer some questions:
🔹 Little research could be found for females aged between 35-50 and >70 years old, highlighting the gaps, particularly for women & AFABs in the PERI menopause life stage. 
🔹 Limited evidence was available for 1 or >4 training sessions per week, highlighting the gaps for low & high frequencies & volumes.

The takeaway messages: 
🏋🏻‍♀️ Independent of age, consistent resistance training builds strength & reshapes body composition, generally. 
📝 Individualisation remains important, over strict sex- or age-based distinctions.

I've been absolutely blown away by the response to the Neurodivergent Health Summit for Pracs - we sold over 30 tickets in just 24 hours, which is phenomenal!

Let's chat about one of the sessions that I'm really looking forward to.

@rhihardingham is an experienced naturopath, presenter, educator and author, focusing primarily on reproductive and gynaecological health - but honestly - does she even need an introduction? Maybe I should just say she's an ABSOLUTE LEGEND 👏👏👏

Fun fact: Rhi and I actually went to college together (SSNT alumni 2005). Over the last 20 years I've watched her become THE go-to practitioner and educator on women's/AFAB health and hormones. 

More recently, she's also been down the rabbit hole of ADHD and neurodivergence, which is why we're in the best possible hands for this session.

Rhi's presentation will show you her take on the intersection of ADHD, oestrogen, dopamine and perimenopause. She will explain the interplay between the brain and hormones in a way where you will "get it." 

You will leave with a deeper overall understanding, new tools to support your clients, and knowledge of how to work within an integrative healthcare team in order to best support your midlife ADHDers. 

The session is delivered in neuro-affirming framing, positioning ADHD as a lifelong neurotype rather than as a pathology (we love that!)

The Neurodivergent Health Summit for Pracs is happening on Fri 20th and Sat 21st November, from 9am - 1:30pm each day.

Another month, another conference 🫠 

This time, the Australasian Menopause Society Congress, and it was a packed house: 700 with 300 on the waitlist…. Menopause really IS having a moment! Fabulous to see so many GPs enthusiastic to up-skill in this field. 

In particular it was great to hear from Prof John Eden on breast cancer (punchline- it’s not caused by endogenous or body identical hormones), Prof Amanda Henry on why pregnancy history is such an integral consideration for clinical midlife management (complicated pregnancy = increased need for comprehensive peri healthcare), and Prof Robin Daly on exercise as (essential!) medicine for menopause (want to age well? Exercise is not optional). 

Shout out to @alm_natural_medicine (with @nicm_au) for presenting on complimentary therapies in menopause. Always there to represent us at medical conferences ❤️✊🏼

And sadly… two lowlights of note. 1) A senior male professor saying “if women exercised and ate well we wouldn’t need menopause clinics” (big laugh from the female-dominated crowd- medical misogyny is alive and well). 2) Repetitive (and in my opinion deeply disrespectful) derision of doctors going out of their way to find solutions for routine challenges conventional protocols currently fail to resolve while gaps in research lag (a bit of common sense using safe interventions goes a long way here- as with my fertility patients, I prefer to not wait until it’s too late for them individually. Huge shout out to the many specialists and GPs prioritising patient care right now). 

But of course the best bits are always the networking- so fun to spend a couple of days with our friends and colleagues Caitlin O’Mahony and @dr_tash_the_fanny_mechanic 🥰 Like-minded collaborative care across modalities is golden, what patients are searching for, and what we’re constantly practicing 💫

For nats, highly recommend attending medical conferences. One of the questions I’m asked most by mentees: “Why did my patient’s doctor say that?” These events give the context that provides a clear answer… and we think seeing it for yourself is invaluable. Join us next time 😘 by @rhihardingham
