At 6 months, every sun-damage treatment looks like it worked.

At 30 months, only one still does.

In a head-to-head durability dataset, high-density CO2 laser was the only field treatment still holding up to a 90% reduction in pre-cancerous lesions at 30 months.

Photodynamic therapy, low-density CO2 laser and 5-FU cream all rebounded.

Here's why.

The spot you can see is the smallest part of the problem.

For every lesion that surfaces, the skin around it is hiding far more damage that hasn't shown up yet. The photodamaged field.

Freeze one spot, and the field keeps making new ones.

So in clinic, the sequence matters more than the tool:

1️⃣ See it.
Multi-spectral imaging maps the UV damage hiding under normal light.

2️⃣ Treat the field.
High-density CO2 laser resurfacing rebuilds collagen and replaces damaged cells across the whole area, not spot by spot.

3️⃣ Protect it.
Daily SPF, DNA-repair skincare and internal support. In the ONTRAC trial, oral nicotinamide cut new non-melanoma skin cancers by about 23% at 12 months.

But here's the honest part.

This is a representative durability dataset, not a meta-analysis. Individual results vary. And none of it replaces sunscreen or regular skin checks.

After my own skin cancer, this was never cosmetic to me.
