74 weeks. That is the median Duration of Untreated Psychosis (DUP) in the United States, as established by the landmark NIMH RAISE trial.

The RAISE trial also revealed how critical care is early in illness. Patients with a DUP under 74 weeks experienced both significantly better clinical outcomes, and a higher quality of life. 

Every week represents a compounding risk. When a first episode of psychosis goes untreated, the @taccommunicate reports that the clinical and neurological impacts include neurotoxicity, increased symptom severity, reduced treatment response, and diminished insight. 

The delay doesn’t happen because families don't care. It happens because of deep systemic stigma, fear, and a lack of community awareness regarding the early signs of Serious Mental Illness (SMI).

Early detection and immediate treatment halt this cascade. By championing Coordinated Specialty Care (CSC) networks that integrate medication, therapy, and family support, we can compress the DUP and give individuals with psychosis the trajectory of recovery they deserve.

Caregivers of people living with mental illness carry more than responsibility — they carry emotional strain, isolation, and often put their mental health last to care for others.

While those we care for are not a burden, the impact of caregiving and it's many components can be. Many caregivers shoulder these obstacles for exactly that reason - because their loved one is not a burden and they would do nearly anything to help them in their recovery.

📊 Research shows that about one-third of caregivers experience high caregiver burden, including depression, anxiety, and reduced quality of life.

And yet, caregiver support is still underfunded, overlooked, and under-prioritized.

That’s why we’re starting 2026 with a clear goal:

Ever feel like you are constantly on edge?

Hypervigilance is more than simply being observant. It’s a heightened state of alertness where the brain continuously scans for potential threats, even when there is no immediate danger. You may notice every sound, monitor other people’s expressions or tone, startle easily, or have difficulty relaxing.

Hypervigilance can occur with trauma and PTSD, anxiety, and other mental health conditions.

Understanding hypervigilance can help us recognize that difficulty feeling safe isn’t always a choice. Sometimes, the brain’s alarm system doesn’t know when to stand down.

In light of posting so much serious content for September as Suicide Prevention and Awareness Month, we thought it would be prudent to end the month on a high note and explore a little self-care!

We are all familiar with the concept of doing things for ourselves, but self-care isn't always a day spa, a vacation, or even a perfectly curated morning routine. Sometimes, it's the basics: eating, sleeping, setting a boundary, asking for help, taking a break, or making an appointment you've been putting off.

Self-care is les about doing everything "right" and more about consistently making space for your own well-being. And it doesn't have to be elaborate to matter as long as it is appreciated with meaning!

For caregivers especially, caring for yourself isn't selfish. It's part of sustaining the capacity to care for someone else.

What happens when someone feels disconnected, like a burden, and does not fear death?

The Interpersonal Theory of Suicide (IPTS), developed by psychologist Thomas Joiner, offers one framework for understanding how suicidal ideation and risk may develop across the three concepts of thwarted belongingness, perceived burdensomeness, and acquired capability.

Importantly, these experiences don't tell us whether someone will attempt suicide. Suicide is complex, and no single theory can explain every person's experience.

Understanding these concepts can, however, help us recognize the importance of connection, belonging, support, and professional intervention in suicide prevention.

Perceived burdensomeness is a psychological concept in the Interpersonal Theory of Suicide that describes the belief that one is a burden to others or that loved ones would be better off without them.

These feelings may involve believing that one's needs, struggles, or circumstances negatively affect others, and even when loved ones don't see the situation that way.

Perceived burdensomeness matters because it can contribute to feelings of shame, guilt, hopelessness, and social disconnection. Recognizing these thoughts can help us better understand emotional distress and identify opportunities for support and connection.

A reminder: feeling like a burden doesn't mean you are one. Everyone needs support sometimes and needing help does not diminish your worth. You are loved.

Suicide contagion is an important but often misunderstood concept in suicide prevention.

Exposure to another person’s suicide or suicidal behavior does not mean someone will become suicidal; however, for some vulnerable individuals, exposure can contribute to increased suicidal thoughts or behaviors.

This is one reason how we talk about suicide matters. Sensationalized coverage, excessive detail, or repeatedly focusing on the circumstances surrounding a death can potentially increase risk. Responsible conversations can instead focus on prevention, support, treatment, connection, and recovery.

For caregivers, clinicians, educators, and communities, understanding suicide contagion can help us respond thoughtfully, and especially following a suicide or within communities experiencing a cluster.
