Is the clot risk from hormone therapy about the hormone, or how you take it?

For more than twenty years, blood clots have been one of the biggest reasons women hesitate to start hormone therapy. That concern traces back to a 2002 trial (the WHI trial) that tested one estrogen pill, at one dose, in women who averaged 63 years old.

The finding was real. The problem is, it has been applied to patches, gels, different formulations (ex. estradiol), and doses that trial never studied.

There has long been a biological reason to question that leap. A pill passes through the liver first, and the liver responds by making more clotting proteins. Estrogen through the skin skips that step.

A new study of over 1 million women finally put pills, patches, and no hormones side by side. Part 2 breaks down what researchers found, including where the study falls short.

Yesterday was a beautiful Brooklyn morning to walk miles for menopause!  @mpoweredmenopause splashed the park in pink as Selfority moved with so many inspiring women and men to help fund menopause research.
Hearing from @lindabrosenthal_67 before we walked was inspiring and advocating with @lcb.mcdonald is heart warming.  But, our biggest thank you goes out to our Selfority team members who  supported us in person and virtual.  @rkuramoto, @jamie_wilhelm_leadership, @halliemcope, @gkthomas10, and @naowellness to name a few!

Spent an hour last week with the graduate clinical nutrition students at UT-Austin talking all things menopause, and I am still smiling about it.

These are the people who will sit across from women in midlife for decades. They asked about bone, muscle, sleep, metabolism, and how to talk with patients when the evidence is still catching up. Sharp, curious, and ready.
Menopause barely shows up in most health training. Getting to help close that gap for the next generation of clinicians is the best kind of work.

You're mid-sentence and the word is just gone. A word you've said a thousand times before, and your brain won't hand it over. 

I wrote about this for @katiecouricmedia last week, timed to World Perimenopause Day, because I've lived it, too. A few months ago I reached for the word "uterus" mid-article, a word I've typed 40,000 times in my career, and it simply wasn't there.

Here's what's happening: estrogen fuels the parts of your brain that handle memory and word retrieval. During perimenopause it doesn't just drop, it swings unpredictably before it settles, and that turbulence is what shows up as brain fog. For most women, it resolves once hormones stabilize.

Midlife is also a real window for long-term brain health. A new analysis of the US POINTER trial found that a structured plan (supervised exercise, MIND diet coaching, cognitive training) measurably slowed early markers of brain tissue damage in adults under 70. Past that age, the same plan made no measurable difference.
