The Hidden Impact of Chronic Stress on Evening Cortisol and Women’s Health

Dr. Shoma Datta discusses a powerful research example by Cicciolo and Lizar, showing how chronic psychological stress, particularly the invisible mental load of managing a household, keeps evening cortisol elevated when it should drop. This disrupts sleep architecture and suppresses reproductive hormones, leading to clinical symptoms like unexplained weight gain, fatigue, and brain fog in women in their forties.

If your doctor seemed nervous about prescribing hormones… this is the reason.

In new patient consults, one of the first things we talk about is her last doctor — don’t assume it was a bad doctor.
The truth is, hormone therapy sits at a tough intersection of outdated training, evolving research, media confusion, and appointments that are simply too short to untangle it all. Of course doctors feel nervous. Of course patients feel dismissed. Nobody designed it this way — but here we are.
So this visit is a reset for a fresh start.
My goal with every new HRT consultation is two things:

1️⃣ You feel good every single day — sleep, energy, mood, symptoms addressed

2️⃣ We use hormones strategically to build long-term health — heart, brain, bone, metabolism — your overall longevity
Hormone care done well isn’t just symptom relief. It’s a longevity strategy. ♥️🧠🦴⚖️

If you’ve felt stuck or dismissed, it’s not too late to start over.

📣 PEPTIDE POLICY UPDATE:

HHS has released an Interim Enforcement Discretion Policy for compounding the six peptides that FDA’s Pharmacy Compounding Advisory Committee recommended in July 2026.

The six peptides and their covered routes:

BPC-157: subcutaneous, intramuscular, oral, topical
TB-500: subcutaneous, intramuscular
Epitalon: subcutaneous, intramuscular (intermittent courses)
MOTS-c: subcutaneous, intramuscular
Semax: intranasal
KPV: oral, topical

What it does and doesn’t mean:

The policy establishes a temporary enforcement discretion framework:

✔️ HHS does not intend to take enforcement action against state-licensed 503A pharmacies, federal facilities, or licensed physicians who compound them under defined conditions while FDA completes formal rulemaking
✔️ Bulk substances must come from FDA-registered manufacturers with a full certificate of analysis
✔️ Single-ingredient products only. No blends.
✔️ Each peptide is limited to the routes listed above
✔️ Serious adverse events must be reported to FDA through MedWatch
❌ This is NOT FDA approval
❌ It is interim and can be revisited if safety or quality concerns arise

HHS describes the policy as a way to move patients away from unregulated research-use-only products toward a more controlled supply. As a physician, I see that as a meaningful step toward safer access, as long as it comes with real clinical oversight.

The internet is loud about peptides, hormones, and “optimization.” Very little of it comes from clinicians on the front lines of longevity.

On October 14, at @thenednomad, @wellnessgoth will be moderating a panel called “The Midlife Reset”, which brings together four voices to separate hype from what’s working in clinical practice to help people look and perform their best in midlife: how and when peptides are used safely, hormone replacement therapy, and the other advanced protocols worth considering.

Grateful to share the room with @drpoonamdesai, @doc.meesh, @tywheezy, and @shomadattamd.

With support from @creiadrinks, @gameday_manhattan, @systemlabsinc, @sumerohealth, @chief.nutrition, @engineerednutrition.us, @drinkalya, and @tmw.longevity.

@shomadattamd dropping knowledge!!

Dr. Datta is not just my sis and respected colleague, shes also a board-certified obstetrics and gynecology physician who residency trained at Johns Hopkins and completed a post residency surgical fellowship at Icahn School of Medicine at Mt Sinai and is a certified menopause provider. She was medical director at one of the first longevity clinics in the world with @dranantmd (current CMO at @hims), advises for health tech and VC funds and her day job is running clinical operations at @nexthealth 🔥🔥

🌟 FDA & SENATE EYES & EARS ON MENOPAUSE IN WASHINGTON DC!

As a board-certified OBGYN - menopause & longevity specialist, my daily mission is to help my patients navigate mid-life with vitality. For far too long, misinformation caused an entire generation of women to miss out on timely, appropriate menopausal care—including life-changing hormone replacement therapy.

Optimizing the phases of menopause isn't just about managing hot flashes. It is a critical component of healthy aging, offering vital cardiovascular defense, bone density preservation, and cognitive protection as we get older.

This week marks a massive turning point. We aren't just looking backward at what was missed; we are looking forward to monumental milestones happening in Washington: LINKS IN BIO!

🗓️ Wednesday, September 16, 2026 3:30pm ET: The US Senate is convening the first-ever congressional hearing focused entirely on menopause care. This historic event will be broadcast live on YouTube, shining a national spotlight on the medical care gap.

🗓️ Thursday, September 17, 2026 9a-4:30p ET: The FDA is hosting a public workshop on Testosterone Use in Menopausal Women. You can attend this landmark event virtually via Zoom or watch on YouTubeLive. We know testosterone is a key driver of female metabolic health, mood, and long-term vitality, yet we still lack an FDA-approved formulation specifically titrated for women. This meeting is the first major step to fix that.

🗣️ YOUR STORY IS THE PROPULSION FOR CHANGE.

The FDA has already opened a public commentary docket (Docket FDA-2026-N-5479) through October 19, 2026. If you have been denied HRT, struggled to find a knowledgeable provider, or had to advocate endlessly for your health or simply have anything to say on the issue the government is finally asking to hear from you.

🔗 Take action today: Tap the link in my bio to get the YouTube Livestream Link for Congress, register for the FDA Zoom Meeting (fda.gov/consumers/womens-health-events/fda-public-meeting-testosterone-use-menopausal-women-09172026), and directly submit your comment to the federal register.

The Hydration Rule That Changes Everything

Dr. John Thomas talks about the critical importance of proper hydration, starting with half your body weight in ounces of water daily, and why liquid calories from soda, juice, and coffee drinks can sabotage metabolic health.

I started that day full of nerves as a medical student not knowing the true devastation that lied ahead for us all. I remember seeing & smelling the smoke across the river from NJ. I remember the disbelief of a whole country hurting and lost. I only hope we can honor the memory of those who perished by being our best as Americans - together & everyday 🙏🏽.

Talking peptides with my dear friend @shomadattamd VP of medical operations @nexthealth

Episode drops soon! This was a fun one, no script, early AM before both of us had to run to drop kids off to school but we still made it work because we love medicine and making sure patients are getting their information from the right sources.

Btw there is a 10-15% subset of patients that even well studied GLP-1’s don’t work on!
