Hi I’m Paul, and I’ve never done one of these posts. Long overdue.
.
This is my physical therapy clinic, my only social media page, and my primary passion in life.
.
In January of 2020 (yup), I started my own practice because I was tired of being underwhelmed by therapy. I wanted to start a clinic that emphasizes education and progressive exercise as primary treatments, where time can be spent to reach rehab goals, which is led by objective measures as often as possible, and where I don’t feel obligated to fill some visit count quota. I learned the business skills along the way, and am so proud that after 6 years, it’s working.
.
I believe PT is best described as an applied science. We need to use our best available evidence, tools and interventions to solve a complex human problem. Sometimes the solution is simply time, sometimes it’s surgery or medicine, and often it’s rehabilitation.
.
While I will see almost any orthopedic or musculoskeletal condition, the majority of my weekly practice targets recovery from complex surgery, major injuries, and sports related conditions.
.
As a solo entrepreneur, I am the business. I try to be as genuinely me as I can. And I hope this Instagram page reflects that. As a double board certified PT (ortho and sports) I take what I do seriously. As a goofball, I make sure to have fun along the way.

Sometimes a referral to a pro is absolutely warranted.
.
Sometimes time is precious and the intervention is reasonable enough to tackle ourselves.

1) order carbon fiber plate
2) compare to shoe insole and mark edges to trim
3) grind with high grit flap disc, in an open area WITH PPE, this stuff’s nasty

These are my post-op knee non-negotiables.
Yours could be different.
.
Background, rationale, and methods:

1) post-op knees avoid load. Everything in their healing tissues and neurology avoids it. They shift away, keep the knee straight, they shift to their calf and lean with their trunk. The key to rehab is regaining tolerance and performance through the knee extensor mechanism. Training it. Show the body it can do it and reclaim it.

2) constraint based training. If we want to push an adaptation, constrain the activity so they must use the chosen strategy. Walls are great for that.

3) bend your knee, preferably out in front of you.

The Exercises:
1) Foot on bench mobility. Fed/back, and clockwise/counterclockwise
2) Wall facing single leg squat
3) Single leg wall sit (of course)
4) Wall slide squat
5) Single leg wall slide squat
6) knee over toes squat walk on toes

Complexity usually just means more time and gradual progression.
.
The principles rarely change.
.
My new phrase is “get your vanity from your quads. Spend the summer flexing those thunder thighs at the beach.”

During a session of the usual post op knee stuff at 7 weeks (quad work, cryo, ext ROM mgmt, motor control), we add in some hamstring training and workshop how to get it in with his gym equipment.
.
1 day later I get this video asking if it’s alright
.
Hmmmmmmmmmm I had to think.
.
Usually this kind of condition would have tons of reluctance and pain with kneeling. I wouldn’t usually think of bringing in Nordic curls until at least 3 months. Even more commonly I’d bring them in around month 5. Doing this could flare up his PFJ. Not particularly risky to the grafts, but is it worth the risk to flaring up the knee?
.
But who am I to arbitrarily slow down a patient if it’s safe and they say it’s tolerated, just cause it seems. Early or risky to a flare up?
.
So I split my quandary down the middle. Once a week, more traditional hammy work on other days.
.
Let this be a lesson: go hard on the simple stuff in the first 4-6 weeks.
-edema control (elevation, compression)
-pain mitigation (icing)
-gentle ROM, ext emphasis
-Assistive Device as needed (gradual reduction)
-f@ck braces
-take time to recover
-always quad
-always educate

Nothing wrong with some band exercise, I keep a roll around the clinic and use it weekly.
.
But I have a few qualms that limit using it.
1) super easy to under dose, very rarely does someone get near proper fatigue. You can always do 1 more.
2) it’s progressive resistance, not consistent through the range. More tug = more pull
3) the body knows it’s weird. This comes best from nasa studies on exercise in space. Physiology responds best to the resistance of gravity.
.
Shoulder external rotation strength is like money, if you can get some more you should get some more.
.
This champ is 6 months out from labral repair and biceps tendodesis. He’s going great, but still 75% shoulders ER strength deficit (35.5 vs 47 lbs).
.
So we’re using 20lbs against gravity.
.
Don’t think we would have captured that deficit with bands.

2024 was a winner.
.
It’s been a great maturation into a business that works.
.
Hired a rockstar new operations manager, paying down bills, taking time off, and serving my community with the skills I’ve worked so hard to develop.
.
2025 gunna get wrecked. Good luck, new year. See you in the therapeutic thunderdome.

Simple solutions with simple tools.
.
I’ve been using my gait belt for all sorts of dynamometer testing and isometric training.
.
After 4 years of use, it’s time to put this one out to pasture.
.
Pretty remarkable how durable these things prove to be. Years of daily stress. Hundreds of pounds of force. It just kept going.
.
Simple solutions built on our best principles, to resolve people’s problems.
.
Shout out to @tindequipment for making a great dynamometer for everyday use.
