What happens when conventional herpes treatment fails? 🦠

Acyclovir, valacyclovir and famciclovir have long been the mainstay of HSV treatment. But in immunocompromised patients, refractory infections can be particularly challenging to manage.

Enter PRITELIVIR ! A first-in-class oral helicase–primase inhibitor with a completely different antiviral target.

With its recent FDA approval, pritelivir introduces a new treatment option for refractory mucocutaneous HSV infections in immunocompromised adults.

And the Phase 3 results are worth discussing:

📊 63% vs 34% complete lesion healing by Day 28 compared with investigator-selected treatment.

But what makes pritelivir different from acyclovir? Can it overcome antiviral resistance? And where does it fit into the current HSV treatment algorithm?

I’ve broken down the mechanism, clinical evidence, dosing, safety and clinical positioning in this carousel.

ID Doctor Responds to Appropriate/Inappropriate ID consults 🦠👩🏻‍⚕️💊

Long video compared to my usual but if you ever have to consult ID, I think it might help to watch!

Thanks to @hospitaldoctort for the original video. If you’re not already following him, you should!
Teamwork makes the dream work and the patient benefits from that!

My take:
📌Don’t consult too early and don’t consult last minute
📌Absolute yes to ID consults for SAB, candidemia and complex scenarios that require prolonged therapy
📌Yes to starting the work up for the patient to get everything going

ID Friends: what are some things you would add?

So, 8/31 is International Overdose Awareness Day and many people don’t know this…so over the next few days I’m going to share a few posts on overdose recognition, prevention, and treatment.

Let’s start with opioid overdose!

We still see it far too often in our communities, EDs, hospitals, and ICUs. And with today’s increasingly unpredictable and contaminated drug supply, all of us should know how to recognize it and what to do next.

So…let’s talk naloxone.

We should be just as comfortable teaching naloxone as we are prescribing any other lifesaving medication.

Overdose deaths have been moving in the right direction. Let’s keep pushing that number down.

Save this one, share it with someone who should know how to use naloxone.

What’s one thing about naloxone you think we still get wrong?

You asked for it, and here it is!

Managing alcohol use disorder doesn’t have to be complicated. These are some of the most common questions I get about treating AUD and ….I even added a Practice Shift flowchart for those of us who like to see it all laid out visually.

Hopefully this makes things a little easier the next time you’re sitting with a patient with AUD and thinking, “Okay…where do I start?”

Share this with a colleague, friend, or anyone who might find it helpful!

What questions about AUD do you still have? Drop them below …I may make a Part 2.
