Relapsed retractive ortho case! No - this is NOT an airway 💨rant 🗣️ However, I will say that when there’s an obvious tooth-size discrepancy, the patient benefits enormously ( esthetically AND airway wise) when the referring dentist and orthodontist collaborate on a cohesive treatment plan from the outset. If the referral just says “do ortho” 😂 and the orthodontist does - Well, THAT is not interdisciplinary dentistry. That is hardly even 🤷‍♀️disciplinary 🫣 Let’s see what we were working with on this Sweet gorgeous pt 💖 Asymmetric space distribution, asymmetric gingival levels, occlusal cant, canted midline, each incisor had a different axial inclination, highly characterized translucence. Patient said - I will not re-do Ortho. And don’t want my teeth drilled. Oh - and I want a natural smile that looks like me. So we did what she asked for.

The SHORT Case Of The Day! Tooth #14 - Crown With Previous RCT Patient came in with discomfort on tooth 14 in January 2026. The 2D PAX did not show any signs of infection. A LFOV 3D CBCT was taken. There was MSEO present in the sinus and bone breakdown on the palatal root. Patient was given the option of a retreatment or an extraction and implant. He decided to wait to make a decision. He came back to our office recently. He said his crown became lose and now he wanted his tooth fixed with a retreatment. I performed a thorough exam on this tooth. I felt some mobility in which he did not have previously. The crown was not lose. There was a root fracture. The reason why he was not in significant pain was because he had a previous root canal. He will now require and extraction and implant. Don't get me wrong, your procedure can still be done, and you won't remember anything and will be comfortable, but it might be more of a rodeo. See below! A few facts before we saddle up: 💉 Regular cannabis users may need more anesthetic medication during oral surgery—being relaxed doesn’t necessarily mean being easier to sedate. ❤️ Smoking marijuana can temporarily increase heart rate and blood pressure, and smoke can irritate the airway. 😬 Regular users may experience MORE pain and nausea after surgery. 🍬 Yes, edibles count! Tell your anesthesia team what you use, how much, how often, and when you last used it. That information helps us plan your care. There’s a side of implant dentistry we don’t talk about enough. What happens when things don’t go according to plan? On November 12–14, I’m bringing dentists back to Vineland, NJ for our Live Implant Surgery Course, and this time I’m excited to welcome Dr. David Wong as our guest lecturer. Dr. Wong is going to take us beyond simply placing an implant and into one of the most important conversations we can have as clinicians: implant complications. Why did it fail? Was it biologic? Surgical? Restorative? Esthetic? Can the implant be saved? Can it be revised? Or are you looking at a situation where removal is the right decision? We’re going to get into malpositioned implants, compromised and failing implants, peri-implantitis, poor esthetics, hard- and soft-tissue reconstruction, and the clinical decision-making behind managing these cases. But more importantly, the goal is understanding why complications happen in the first place and developing strategies that can help prevent them. How do you treatment plan differently? How do you reduce surgical and restorative complications? How do you communicate with the patient when something does go wrong? And how do you build a more predictable path toward long-term implant success? Then we take education out of the lecture room and into the operatory. Friday and Saturday, you do the surgery. Under the guidance of our faculty, doctors perform live implant surgeries on actual patients, with cases tailored to their experience and goals.
