Cancer can leave clues in the blood before progression appears on scans.

Circulating tumor DNA, or ctDNA, consists of small fragments of DNA released by cancer cells into the bloodstream. By analyzing these fragments through a blood test, we can look for genetic changes that may explain why a treatment is becoming less effective.

SERENA-6 asked an important question: Can we use that information to change treatment earlier and improve outcomes?

The study focused on people with ER-positive, HER2-negative advanced breast cancer receiving an aromatase inhibitor plus a CDK4/6 inhibitor. Blood tests every 2–3 months looked for emerging ESR1 mutations.

Why ESR1? This gene makes the estrogen receptor. Certain mutations can keep the receptor active even when estrogen levels are low, allowing cancer to become resistant to aromatase inhibitors.

When an ESR1 mutation was detected, before progression appeared on scans, 315 patients were randomized to switch their aromatase inhibitor to camizestrant or continue their existing treatment. Both groups continued the same CDK4/6 inhibitor.

With longer follow-up:

• Median time to first progression or death: 16.8 versus 9.2 months.
• Median time from randomization to progression on the next treatment or death: 25.7 versus 19.1 months.

Both results favored the camizestrant switch. An overall survival benefit has not yet been established.

Three years of treatment. A benefit still evident at six years.

New results from the NATALEE trial, reported at ESMO 2026, show that adding Kisqali (ribociclib) to endocrine therapy improved overall survival in patients with HR-positive, HER2-negative stage II or III early breast cancer at higher risk of recurrence.

What does that mean?

Compared with endocrine therapy alone, Kisqali plus endocrine therapy was associated with a 19.1% relative reduction in the hazard of death. At six years, the absolute survival difference was 2.1 percentage points, or approximately 2 more people alive for every 100 treated.

Benefits also continued in preventing invasive disease events and distant recurrence, years after Kisqali treatment ended.

Kisqali is not appropriate for everyone. Discuss your recurrence risk, potential benefit, side effects and monitoring with your oncology team.

As you age, you def need more than aerobic exercise only. Strength/weight training helps you to get stronger and strengthen the core which helps to keep you from falling!!!🩼

Move.
Do something.
You’re gonna feel so much better.
And you may even lose some weight, which I need to do cause this year of healing has been rough on a Sistah!!👀
