Another day another disc replacement. This time it was in a gentleman who had degenerative disc disease at C3-4 causing nerve root and cord compression. He presented with neck pain arm, pain, and difficult walking (off-balance). This cord compression is called cervical myelopathy and patients typically present as having difficulty walking. This can be quite concerning to an individual and in these scenarios, conservative treatment is not an option. Surgical decompression is the most effective way to alleviate the pressure off the cord. After a thorough decompression we replaced the disc with an artificial disc which will allow him to keep up with his routine lifestyle. He was discharged from the surgery center a couple hours after his operation and recovering well.

Finally had a whole weekend off after 3 weeks on. It’s important to give your body and mind decompress and recover. Now we are going into the week with some inpatient surgeries starting today: 3 level and 4 level disc replacements and a posterior multi level lumbar fusion. Tomorrow is a mix of decompressions, disc replacements, and a large thoracolumbar fusion. Then onto clinic on Wednesday before flying out for a business trip. Having a rigorous schedule isn’t something to boast about, but more so finding a balance in which you can do those things efficiently. I’m glad I took the time to relax the past couple days before the month ahead and all the good things that are happening.

Joe and I have been operating together for nearly a decade and a half. Dr Vanderlinden is a Vascular surgeon (spinal access) and does the exposure and closure of our anterior lumbar cases whether it’s a fusion/disc replacement, and some complex cases as well in the hospital. The expertise he brings to the OR ensures a higher level of safety for patients as we navigate major blood vessels and organs.

Yesterday we performed a two level fusion from L4-S1 on a patient with low back pain and leg pain . This impacted the patients lifestyle and even his golf game.

In this patients case the surgery was performed in the outpatient setting in approximately 1 hr and he went home the same day. While the fusion healing time has a longer recovery period it was the appropriate treatment for his condition that would allow him to resume his activities, unrestricted within a few months.

When you step back for a moment you get a different perspective of the patient. It’s not just about treating the patient’s spine pathology but we are treating their lifestyle, their job, their family life. Every single day we see patients who afflicted from a myriad of symptoms because of degeneration in their spine. Those symptoms can impact their mental and physical well being and ultimately their ability to function on a daily basis. My approach with all patients is to optimally return them to their family life, work, and activities they enjoy.

Trigeminal Neuralgia is a condition which causes excruciating pain in the face. This is most commonly caused a small artery beating against the trigeminal nerve which is the nerve with three branches and provides sensory input from the face.

This is initially diagnosed via MRI and treatment includes a variety of medications, including Botox. If that fails , we can attempt Gamma Knife Stereotactic Radiosurgery. The last option is to perform a Microvascular Decompression. A nickle sized hole gains us access to the brain stem area and a small pledget is placed between the artery and nerve. Patients tolerate the patient well and the entire operation is performed under computer guidance using BrainLab.

Dr. Mesiwala was recently featured in an article regarding Chris Hemsworth’s discovery of increased Alzheimer’s disease risk. He discusses the disease process, the value of genetic testing, understanding how that information should be interpreted, and what changes can be made in our daily life based on genetic testing.

The main thing is that there is value to genetic testing, as long as you understand what it is good for and how the information should be used. Information is clearly power, and in an age where we can stave off disease, this may be the ultimate gift for someone this holiday season.

With a couple of lumbar TDAs (total disc arthroplasty / artificial disc replacement) on the schedule today, I wanted to highlight the person that makes it happen. Dr Vanderlinden is the vascular surgeon I’ve worked with for many years. He’s a highly skilled and experienced surgeon.

Dr Vanderlinden is integral to the operation as the person who creates a corridor to the spine via an anterior abdominal approach. He moves aside the intestines and major blood vessels. Once this is done and I complete the decompression + implant, he proceeds to close everything.

Dr Vanderlinden and I have performed many anterior lumbar cases and a multitude of other complex spinal operations together. It’s always a pleasure to work with this rockstar.

Watch the last clip to see how the retractors are placed and see the disc space.
