🦷 EXTREME WEAR | WHEN DIAGNOSIS DRIVES THE RESTORATION

A challenging case of advanced anterior tooth wear, with a combination of erosion and attrition.

🔎 THE CHALLENGE
Chronic wear cases often present with dentoalveolar compensation and a significant reduction in restorative space — without necessarily losing vertical dimension.

Recognising this distinction is critical for diagnosis and treatment planning.

🧩 INTERDISCIPLINARY PLANNING
With severe wear, orthodontic realignment can be necessary to optimise tooth position, followed by crown lengthening to establish sufficient remaining tooth structure for predictable restoration.

❤️ PRESERVING VITALITY
Despite the advanced wear, the goal was to maintain pulpal vitality wherever possible.

During the healing phase, the central incisors were protected with direct composite, as preparation was already very close to the pulp chambers.

✨ THE RESTORATION
Once the tissues had healed and the foundations were established, the teeth were restored with full-coverage ceramic restorations, beautifully crafted by Matteo, whose attention to detail was exceptional.

🛡️ PROTECTED FOR THE LONG TERM
The patient is now comfortable, with the occlusion carefully idealised and protected with a splint.

The goal was never simply to restore worn teeth — but to rebuild them with biology, function and longevity in mind.
