Interesting case report reminding us that cefazolin can induce coagulopathy, particularly among patients with renal or liver dysfunction.
There’s other case reports on this, and a coagulopathy warning is in the FDA label, making me wonder if this is infrequent or under recognized 🤔.
Good paper and something I’ll adopt in my practice (and no, I don’t remember the coagulation cascade of the top of my head, the mechanism is in the paper 😂).

Words and terms matter, and what time I have come to see that asymptomatic bacteriuria does not mean the same to everyone. And by the way, if you know what the term actually means, then I consider you an infectious diseases person 😘😇.
*edit: Bacteriuria in the context of pregnancy is a different story, as treatment considerations differ in this population 😉

We now have four guideline/guidance documents on the management and prevention of C diff.
Clearly different (hot!) takes on many important issues (but overlapping authors with AGA and ACG).
It should be noted that two recent papers on this topic (TAPER V and OpTION) are not mentioned in this document, and I think that is more related to the pace of evidence output, compared to when this type of revisions are usually commissioned, completed and published.

📣POET II RESULTS ARE OUT!
…and they’re a bit disappointing…
The results are a bit of a mix bag because although the safety end-points were met, there was a much higher risk of relapse in the tailored-therapy group.
I’ve seen some criticism about the study design but I’m not a trialist so don’t wanna delve into that, but as a clinician I cannot ignore the relapse rates. Even if it’s a small number overall (and includes at least 5 patients that relapsed twice!) I cannot use this data to confidently shorten antibiotics for IE.

📌 If you don’t know how to interpret or why you’re getting a test, please stop and think before you order.
📌Reflexively ordering certain labs can cause more harm than good especially when ordered inappropriately.
