Before you call it Wellens, check lead III.
Anterior T-wave inversions can appear in both ACS and pulmonary embolism. When lead III and V1 are also inverted, PE and right-heart strain deserve a closer look—especially with dyspnea, hypoxia, or syncope.
Use the distribution to sharpen your differential. An ECG alone cannot confirm or exclude PE.

PDs are hungry for data! And they are being given less and less quantifiable data points as the years go on. The void absolutely should not be filled with research. I do wholeheartedly believe in holistic review of applications but we need to strike a balance or else sometimes it just seems like guessing.

NIGHT SHIFT ADVICE PART 2: Night shift survival is not about being tougher.

It’s about having a system.

When I’m switching into nights, I try to stack the deck in my favor:

✅ Sleep in if I can
✅ Lift after waking, before shift
✅ Eat a real meal before work
✅ Take a pre-shift nap
✅ Front-load caffeine early
✅ Avoid hard workouts after shifts
✅ Keep late-night food lighter and high protein
✅ Use a shorter recovery sleep (4 hrs) after my last night to flip back

You have to manage light, caffeine, food, training, and sleep like they all matter

For me, workouts are usually best after waking and before shift. It gives me energy without wrecking my post-shift sleep.

And when I’m switching back to days after the last night, I don’t sleep all day. I take a shorter recovery sleep (about 4 hours), get sunlight, move my body, and aim for an earlier bedtime with melatonin.

Night shift is never easy.

But a system makes it less brutal.

New lawsuits are being filed against programs with disproportionate amount of foreign medical graduates on their match roster. There are a multitude of legitimate reasons why this could happen and this is a very dangerous message being sent! Illegal discrimination should be investigated, but this is a very slippery slop. In reality many programs have less control over who they match than you would think…Especially ones that are at risk of not filling. Let’s make decisions based on FAIRNESS and not FEAR.

It’s easy to get caught up in the rat race and get discouraged when you fall, especially when you’re surrounded by the highlight reel that is social media. I’m hear to tell you that every successful person has failed numerous times. I took the MCAT twice, got rejected from US MD schools twice, and worked my butt off after @stgeorgesu gave me the opportunity to earn my MD and got killer board scores to match. These earlier failures helped mold me into the person I am today and helped me say “why not me?”. Every doctor, every business owner, every human has failed. KETAMINE OR ETOMIDATE FOR RSI?: NEJM DEC 2025 - @emswami and @docreilles discuss the NEJM trial Ketamine or Etomidate for Tracheal Intubation of Critically Ill Adults. This was a randomized, unblinded RSI trial comparing ketamine versus etomidate for intubation, where the primary outcome (mortality) showed no difference but the secondary outcome of hemodynamic decompensation was higher with ketamine, largely driven by starting or escalating vasopressors. This finding has pushed some clinicians away from ketamine, while @docreilles will continue to use etomidate except in asthma/COPD due to ketamine’s bronchodilator effect. Like the trial showed, he doubts that etomidate’s adrenal suppression is clinically significant in acute care. @emswami usually a ketamine user, thinks the trial is a wake-up call but the unblinded design and differences in pre-intubation pressor use may have biased the hemodynamic outcome #emergencymedicine #medicaleducation #criticalcare #medicine
