When treating severe periodontitis with antibiotics, does it matter when you start them? 🤔

Adjunctive metronidazole plus amoxicillin is widely used with scaling and root planing (SRP) for advanced periodontitis, but the best timing had never been tested in a randomized trial. A new double blind, placebo controlled RCT in the Journal of Periodontology, with authors from our Department of Oral Medicine, Infection, and Immunity, compared starting the antibiotics on the first day of SRP (early) with starting them right after SRP was completed (late).

🔬 What they found:
• In 68 patients with stage III/IV periodontitis, both timings significantly improved all clinical parameters at 1 year, with no serious adverse events.
• A similar share reached the clinical endpoint (≤4 sites with PD ≥5 mm): 58.8% early vs 55.9% late, with similar PD and CAL reductions.
• Both timings reduced red complex pathogens and A. actinomycetemcomitans, but only the early group showed a significant drop in orange complex species.
• In network analysis, beneficial Actinomyces formed an independent, stable cluster in the early group by 1 year, while in the late group they remained entangled with pathogens such as Fusobacterium nucleatum.

💡 Timing did not change clinical outcomes at 1 year, but starting antibiotics on the first day of SRP led to a faster, more favorable shift toward a health associated subgingival community, which may support long term stability.

👏 Congratulations to our HSDM authors Dr. Eduardo de Souza Oliveira, Dr. Shunsuke Matsumura (@s.matsumhu), Dr. Edson de Sousa (@edson.de_sousa), and our department chair, Dr. Magda Feres (@magdaferes), and all collaborators at Guarulhos University and Institute of Science Tokyo!

PerioDash 2026 with the HSDM Perio family! 🦷🏃‍♂️🏃‍♀️

Today, the HSDM Periodontology community came together for our annual PerioDash, getting active, having fun, and raising awareness for periodontal and peri-implant health! ❤️

PerioDash supports the American Academy of Periodontology Foundation and its mission to advance periodontal education, research, and patient care. We’re grateful to everyone who joined us and helped make today such a great one!

Nothing better than bringing our residents, faculty, friends, family (and a few four-legged supporters 🐾) together for a great cause.

See you at the next PerioDash! 🏃‍♂️💨

Rationale is key.

AI, social media, and short-form education give us incredible opportunities to access knowledge, learn, and develop. But technology can never replace the principles, biology, and clinical reasoning that guide our decisions.

This is the philosophy behind the HSDM program developed for the Steigmann Institute — going beyond what you can do but at the same time understand why we do it.

Because techniques evolve and should be adapted, but principles remain.

Understanding the rationale. Advancing clinical practice.

I am very honored to share my appointment as an Invited Lecturer at Harvard School of Dental Medicine.

Returning to HSDM in this role is particularly meaningful to me after spending two years in Longwood , first as an ITI Scholar and later as a Research Fellow & Instructor. During my second year, my clinical and academic work became more focused on the Department of Oral Medicine, Infection, and Immunity and Periodontology, where I developed a particularly close connection with the department, its faculty, and the residents.

Teaching the residents was already an important part of my work during that time, teaching guided surgery, abutment selection, restorative decision-making, and digital workflows.

For my inaugural lecture as Invited Lecturer, I had the opportunity to speak to the first-year residents about the science of implant placement depth, emergence angles, abutment selection, and guided surgery.

I am very grateful to Prof. Magda Feres, Program Director Dr. David Wu, and the entire team at HSDM for their trust and confidence in my contribution to resident education.

It is a great honor to continue my academic relationship with Harvard School of Dental Medicine in this new role.

I'm delighted to share that our research has been featured by the FAPESP News Agency.

In this multicenter, randomized, double-blind, placebo-controlled clinical trial, we found that adjunctive host modulation therapy with omega-3 fatty acids and low-dose aspirin achieved clinical outcomes similar to adjunctive antibiotic therapy in the treatment of periodontitis.

After one year of follow-up, a similar percentage of patients in both groups achieved the endpoint of periodontal treatment, demonstrating comparable clinical efficacy. Interestingly, combining both therapies did not provide additional clinical benefits. Omega-3 and aspirin were administered for six months, while metronidazole and amoxicillin were prescribed for 14 days.

These findings highlight the potential of host modulation as an evidence-based strategy for periodontal therapy.

It is incredibly rewarding to see this work gaining visibility and contributing to the advancement of evidence-based periodontology.
