A stent reopens the artery. It does not cure the disease that closed it.

The blockage was treated. Blood flow was restored. The patient feels better almost immediately. That part is real.

But atherosclerosis is a disease of the entire vascular system, not one segment of one artery. A stent treats the lesion in front of you. It does nothing for the plaque already building in every other artery you have.

The data backs this up. In the COURAGE trial (Boden WE et al, NEJM 2007, 2,287 patients with stable coronary disease), adding PCI to optimal medical therapy did not reduce death or heart attack compared to medical therapy alone: 19.0% versus 18.5% at a median follow up of 4.6 years. Fifteen year follow up data confirmed the same result.

What actually changes your risk after a stent:

Statins, dosed to an LDL target, not just “started”
Blood pressure control
Smoking cessation, completely
Diabetes control
Daily movement and a real change in diet
Antiplatelet therapy taken exactly as prescribed, not stopped early

A stent buys you time and relieves symptoms. What you do after the procedure decides whether you need another one.

Save this if you or someone you love just got a stent and thinks the job is done.

💊 What if treating obesity no longer required an injection?
The future of obesity treatment is opening up new possibilities: GLP-1 medications in oral form.
🔬 Among the new options expected to become available in Italy is oral semaglutide, which has shown significant weight-loss results in clinical trials, as well as orforglipron, a different molecule that acts on the GLP-1 receptor.
But keep in mind: oral does not automatically mean safer or suitable for everyone.
These medications require specialist assessment, a medical prescription, and appropriate monitoring.
🎯 Because the goal is not simply to lose weight, but to treat obesity as a chronic disease, improve health, and reduce the risk of associated complications.
💬 What do you think? Could the option of taking an oral medication make a difference for people who are hesitant about injections?

Severe aortic stenosis with low gradient. Which number do you trust.

A patient’s valve area comes back under 1.0 cm2. By the numbers, that is severe aortic stenosis.

But the mean gradient on the same echo is below 40 mmHg. By the numbers, that looks mild to moderate.

Same valve. Two numbers pointing in different directions. This is called discordant grading, and it shows up in roughly 20 to 30 percent of patients labeled severe AS.

Valve area and gradient are not independent. Gradient depends on flow across the valve. If stroke volume is low, even a tight valve will not generate a high gradient. A low gradient does not always mean a less severe valve. Sometimes it means less flow crossing it.

Before trusting either number alone, check flow. Stroke volume index, left ventricular ejection fraction, and when needed, dobutamine stress echo to separate true severe stenosis from pseudo severe stenosis.

This is why aortic stenosis grading is a pattern, not a single number on a report.

Save this for the next discordant echo you read.

Heart blocks we see them all the time but do we recognize them correctly and what exactly is the importance of each. Save this carousel for future as you are surely going to run into these at some point
