The Pen is mightier than the Sword. Mark before you inject. Usually but not always.

Local anesthetic swells tissue. Once it's in, edges move and the asymmetry you were planning around can disappear. Marks made after injection are marks made on distorted anatomy.

So I mark first, with the patient positioned the way she'll be judged in daily life, and I plan the tissue I'm keeping, not just the tissue I'm removing. Then the block.

Most of the over-resections I see in revision didn't start with a bad cut. They started with a plan made on the wrong anatomy. Them they pulled on tissues as they marked. Totally altering the intended lines.

On the other hand I know where my marks will be before I even mark or cut. I have done so many of these. Local anesthesia wetness can make the blue marks run and go all over the place and disappear. Since I inject only small drops I do not distort the anatomy with my “Microtumescent Anesthesia” technique. Injection first before marking is often the best choice for the most experienced surgeon.

On Gynflix I walk through marking for Rim, Hybrid, and Barbie, and how the plan changes for each. Learn all my tricks if you are a surgeon.

A consult at Alinsod Institute is a conversation first.

We talk through what is bothering you, what you have already tried, and whether an in-office, awake approach even fits your anatomy and comfort level. That can include perineoplasty, vaginoplasty, Hybrid, Barbie, or Rim labiaplasty, clitoral hood reduction, anal skin tag removal, or RF feathering. Sometimes the right next step is menopause care or Hormone Replacement Therapy, not surgery. FaceTime, phone, Zoom, or in person.

I am Red Alinsod, MD. I will tell you plainly if I do not think a procedure is the right match. I will tell you if I can help.

Results vary. This is education, not a diagnosis, and not a guarantee. Individual results depend on anatomy and healing.
