Summary of the 2025 revision strongly supports manual therapy + individualized exercise (flexibility, strengthening, endurance) for people with mild–moderate hip OA and mobility impairment; additional recommendations include dry-needling for myofascial trigger points, patient education/weight-loss collaboration, and selective use of modalities (therapeutic US = optional).

**Manual therapy: Multiple new RCTs and systematic reviews since the 2017 CPG show that joint/soft-tissue manual techniques (including long-axis distraction and mobilization-with-movement) increase ROM, reduce pain, and improve function. Higher distraction forces tend to increase ROM; lower forces may better reduce pain. No safety signals reported in the reviewed trials.

•Exercise therapy: A meta-analysis of RCTs supports exercise (flexibility + strengthening + endurance/aerobic) for short- and medium-term improvements in pain and function. Studies used sessions 30–120 min, 1–3×/week for 5–16 weeks (but higher frequency options are included).

• Surprisingly Dry needling (DN)! New RCT evidence supports DN directed at myofascial trigger points in hip muscles for short-term improvements in muscle extensibility, pain, and function; the 2025 CPG gives a positive recommendation for DN to treat MTrPs.

•Other modalities / devices: Therapeutic ultrasound has mixed/conflicting evidence — may be added with shared decision-making (graded D). Bracing is not first-line (F). Weight loss (5–7.5% target for overweight/obese) is recommended in collaboration with other providers.

Follow for more: @slave2the.most.gracious How to do lumber mobilization followed by lumber spine manipulation step by step. 95% patient will benefit just by mobilization, only 5% actually needs manipulation! We even have clinical practice guidelines that tells us which patient will actually benefit from manipulation. They are 1) No symptoms distal to the knee. 2) Recent onset of symptoms, meaning less than 16 days. 3) Low FABQW score (<19) 4)Hypomobility of the lumbar spine. 5)Hip internal rotation ROM (>35° for at least 1 hip).
If 3 or more of those 5 items were present, the likelihood of success with manipulation increased by a factor of 2.61. With 4 or more items, the chance for success increased by a factor of 24.38 and with all of these items positive, the chances for success increased indefinitely!

If your patient has 2 or fewer of these items, you will have to think of a different treatment approach. Your patient might rather fall into the stabilization category.

Quiz question- What exercise should you give after manipulation? There is one specific recommendation according to clinical practice guideline. @slave2the.most.gracious

The Arabic text for "Allahummaghfirli zambi kullahu dikkohu wa jillahu" is: اللَّهُمَّ اغْفِرْ لِي ذَنْبِي كُلَّهُ دِقَّهُ وَجِلَّهُ. This translates to: "O Allah, forgive me all my sins, both the small and the great". The full dua, including the additional phrases found in some narrations, is: اللَّهُمَّ اغْفِرْ لِي ذَنْبِي كُلَّهُ، دِقَّهُ وَجِلَّهُ، وَأَوَّلَهُ وَآخِرَهُ وَعَلَانِيَتَهُ وَسِرَّهُ. This translates to: "O Allah, forgive me all my sins, both the small and the great, the first and the last, those that are apparent and those that are hidden." The phrase is a supplication (Dua) seeking forgiveness from Allah. It is often recited during prostration (Sujood) in prayer, according to an authentic hadith from Muslim. @slave2the.most.gracious

Reciting Suratul Tahrim verses 9-10 where Allah set an example of the destiny of 3 different wives with 3 different personalities in the Quran.
"Allah sets forth an example for the disbelievers: the wife of Noah and the wife of Lot. Each was married to one of Our righteous servants, yet betrayed them. So their husbands were of no benefit to them against Allah whatsoever. Both were told, “Enter the Fire, along with the others!”

And Allah sets forth an example for the believers: the wife of Pharaoh, who prayed, “My Lord! Build me a house in Paradise near You, deliver me from Pharaoh and his ˹evil˺ doing, and save me from the wrongdoing people.” @slave2the.most.gracious
