Not every identity spiral is OCD. 🌈I’ve been seeing more conversations on social media about “sexual orientation OCD,” and I think we’re missing some important nuance.

If you’re questioning your sexuality, especially while in a long-term relationship, it makes sense that you might be thinking about it A LOT.

What does this mean for my relationship? My family? My future? What if I hurt someone I love? What if I can’t unknow what I’m discovering about myself?

These are BIG questions. And the anxiety that comes with them isn’t automatically indicative of disorder.

OCD is a clinical diagnosis characterized by obsessions (intrusive, unwanted thoughts, urges, or images) and/or compulsions (behaviors or mental rituals intended to reduce distress or prevent something feared).

While OCD can absolutely focus on sexuality or identity, the subject of someone’s thoughts isn’t what makes it OCD. It’s the underlying clinical pattern.

One concept that can help us understand this is ego-dystonic: thoughts that feel inconsistent with someone’s values, desires, or sense of self. This is common in OCD, although it isn’t a requirement for diagnosis.

And here’s where things get complicated: someone can be genuinely exploring their queer identity AND experience OCD. These aren’t mutually exclusive.

As a therapist who works extensively with identity exploration, I worry about how quickly we’re pathologizing the very real anxiety that can accompany discovering something meaningful about ourselves.

You don’t need to be completely comfortable with uncertainty to be exploring authentically. You can spiral, feel scared, revisit the same questions, and still be engaged in genuine self-discovery.
