Truly honored and grateful to receive the @amcollegegastro International Leadership Award.

This recognition is especially meaningful to me, both personally and professionally. As an Afghan American who came to the United States as a young refugee, I could never have imagined the opportunities I would have to work alongside extraordinary colleagues around the world, advance digestive health, and contribute to a profession that has given me so much.

I have always believed that medicine, education, and leadership transcend borders 🌎 Some of our greatest contributions come through the relationships we build, the knowledge we share, and the opportunities we create for others.

My sincere gratitude to the ACG, my friends, mentors, colleagues, and collaborators who have been part of this journey.

Congratulations to Dr. David Fleischer, my fellow recipient of this year's award. It is a privilege to share this recognition with him.

🔎 Upper GI Endoscopy Awareness

When should you consider an Upper GI Endoscopy?

Upper GI endoscopy (Gastroscopy/EGD) allows a gastroenterologist to directly examine the food pipe (esophagus), stomach, and first part of the small intestine (duodenum).

It may be recommended for:

* 🔴 Vomiting blood or black stools — possible GI bleeding
* 🔥 Persistent heartburn or acid reflux
* 🤢 Recurrent vomiting or unexplained nausea
* 😣 Persistent upper abdominal pain
* 🍽️ Difficulty or pain while swallowing
* ⚖️ Unexplained weight loss or loss of appetite
* 🩸 Unexplained anemia
* 🫀 Patients with chronic liver disease — to screen for esophageal/gastric varices when indicated
* 🔍 Evaluation of ulcers, gastritis, tumors or other abnormalities

Is endoscopy painful?

Usually, no. Throat spray and/or appropriate sedation can make the procedure much more comfortable. The procedure itself is generally short.

Why is it important?

Endoscopy can detect disease early, take biopsies, and sometimes provide treatment during the same procedure—such as controlling bleeding or removing certain lesions.

Don’t ignore persistent digestive symptoms. Consult a gastroenterologist to determine whether endoscopy is appropriate for you.

If you can see blood in your stool, the next step is not to do a stool (iFOBT/FIT) test. The next step is answering the question: why is there blood?

A few things worth knowing about FIT / iFOBT:

• The “O” in FOBT stands for occult which means hidden. The test is designed to detect blood that isn’t visible to the naked eye.

• The iFOBT detects blood. It does not detect cancer. A positive result tells you human haemoglobin was detected. It cannot tell you whether the cause is a polyp, cancer, haemorrhoids, inflammation or something else.

• “It’s just haemorrhoids” is still an assumption until the cause has been established. Maybe it is. But an assumption isn’t a fact just because it sounds plausible.

• There is no blood-colour chart for cancer. Bright red, dark red or black can give clinicians useful clues about where in the gut (i.e. location) bleeding may be coming from but colour alone cannot tell you the cause or whether it is cancer.

• A negative FIT isn’t the same as “there’s definitely nothing there”. Bleeding can be intermittent, and the test only measures haemoglobin in the samples provided above a specified threshold.

• So why do we use FIT at all? Because screening millions of otherwise well, asymptomatic people requires something non-invasive, scalable and cost-effective. Used for the job it was designed to do, FIT/iFOBT is an incredibly valuable screening tool.

• And if you’ve previously had polyps removed and were given a colonoscopy surveillance interval, a negative FIT doesn’t erase that history. Screening and surveillance are answering different questions.

The simplest way I think about it:

The iFOBT is excellent for the question it was designed to answer: is there hidden blood in my stool? Don’t ask it to answer a different question.

And if you see blood, don’t assume the cause. See your GP and to get guidance on the next steps to work out why it’s there.
