// Return to Sport with Caro 🏃🏼‍♀️🏃🏼‍♀️🏃🏼‍♀️

While preparing for a trail run, Caro suffered an ankle sprain. We started her rehabilitation right away with one goal in mind: getting her back to running and ready for the big @eigerultratrail! ⛰️

Here we are sharing some insights into the rehab journey, and the process of getting her safely back to running.

Since then, Caro has successfully completed two trail runs, including the @eigerultratrail, two triathlons, including @triathlonbasel, and a half marathon. 

Congratulations Caro 💪💪💪

// One of the lowest hanging fruits after Injury/Surgery is to get rid of muscle inhibition to further rebuild muscle mass and strength 🍇

Here you see significant inhibition as well as loss of muscle mass in a football player. Atm a single calf raise is not possible. The first goal should be clear at this point!

A strong calf then is the foundation for further athletic developement and is crucial if we think about getting back to field training.

We often see people that have failed rehab in the past, because basic boxes weren't ticked ❌

Shaky calf bridges

Bent knee isometrics & floating heel type movements

Yielding isometric holds

Some support to really feel the muscle contract

Early phase of ankle rehab with professional football player @hirata.1076 ⚽️

In professional sports, time is rarely on your side:
Mai injured her ankle towards the end of the season and had to be ready again for the most decisive matches in a very short timeframe.

To get her back on the pitch asap, our goal in early rehab was to preserve as much strength, power and athleticism as possible while respecting tissue healing times. 

Almost always at the edge of pushing it too hard 😅 

Thankfully, @hirata.1076 is incredibly disciplined and brings an outstanding work ethic to every session. That commitment played a huge role in making her return to play such a success. by @tim_moving_physio

//Inside Rehab:

 At the beginning of our rehabilitation plan, pain was the primary limiting factor. Most movements and daily tasks were not possible when we started. The initial goal therefore was simply to find a movement that could be performed consistently with an acceptable symptom response!

We then started with a single exercise: the leg press. Which we trained regulary and progressively.

As pain tolerance, strength, and movement confidence improved over the following weeks and months, we increased task complexity and specificity. Gradually, training shifted toward meaningful activities of daily living: sit-to-stand transfers, protective stepping, getting up from the floor, and other functional movement patterns.

Effective rehabilitation does not have to be complex.  It's about applying the right stimulus at the right time and progressively increasing the demands to restore function.
Wether your goal is to confidentially getting up from the ground or getting back to field sport again.

// The Y-Balance Test is a reliable tool to assess dynamic postural control, lower extremity function, and interlimb symmetry.

This is how it's done:
• Stand on one leg
• Maximal reach with the other limb in three directions as shown
• Typically 3 successful trials per direction
• Normalize distances to limb length for comparison

📊 Use the results to calculate LSI and Composite Score:

Limb Symmetry Index:
LSI = (involved / uninvolved) × 100
Asymmetries > ~4 cm absolute difference in reach distance are associated with increased injury risk in athletic populations.

Composite Score
(Value of each direction added) / (3 × limb length)) × 100
Values around < 94% are considered as cut off values for potential risks (of course context-dependent)

// After a calf muscle injury, professional football player Jana Vojteková joined us in the mid-stage of her rehab journey. 

One of the biggest challenges in rehab is often the transition from gym-based training to sport-specific activities on the pitch. 
So the mission was clear: bridging the gap to the demands of playing the sport!

Together, we prepared her calf for the increasing demands of athletic performance, e.g. springiness, reactivity, without overloading the tissues. 
In addition we made sure the calf muscles are firing while keeping the load relatively low!
 
With great dedication, consistency, and hard work, Jana successfully made her return to football. She  celebrated her comeback with a goal for @fcbasel1893frauen and is now representing the Slovak national team @sfzofficial 🇸🇰🔥

// The Square Hop Test is commonly used as part of a return-to-activity (RTA) assessment following lower-limb injuries. 

The goal is simple:
✔ Complete as many hops as possible within 30 seconds 
✔ Avoid touching the lines

A test is considered to be passed if there is less than 10% difference to the non-injured leg. Ideally you have preinjury data for comparison! 

The test is one exit criteria for the highest RTA Level, which prepares for Sports such as soccer, Basketball etc. But remember: Passing a single test doesn't automatically mean an athlete is ready to return to sport. It should always be interpreted alongside strength, functional performance, symptoms, and sport-specific demands.

// Inside 9 months of conservative PCL rehab 👊

What a journey! This athlete came to us after suffering a PCL rupture during a football match. 
Together, we decided to take the conservative rehab route.

After an intense phase focused on restoring mobility and building strength, we gradually introduced more sport-specific drills and on-field tasks to prepare for the demands of the game again. To get the best Out of it we followed a systematic and progressive rehab approach.

The goal was never just to “feel better”. It was to return stronger, more confident, and ready for competition again. 🔥 by @tim_moving_physio
