one way to help balance our estrogen and other hormones is to partially block production of estrogen if we tend to be ā€œestrogen dominantā€. The main hormone that creates estradiol from the androgen hormones like testosterone is aromatase. We can use food and nutrients to help block aromatase action. And yes, if you have a strong family history of estrogen driven cancers, it would be a good idea to regularly include these foods into your diet.

Night Blooming Cereus (Selenicereus grandiflora) is a lesser known plant that can really be helpful for menopausal anxiety. Its specific indication is for anxiety-produced palpitations and the feeling of ā€œI’m going to dieā€ that comes on for no reason. It can also be helpful in combinations for vasomotor symptoms and menopausal headaches. It’s not native to where I live and I’ve never seen it in person, but I have a friend who grows it in her yard in AZ and she’s sent me pictures of it in blood. Really lovely!

Green tea is a simple way to affect lipids. And if you don’t really like the taste, use it as the liquid when you make your smoothies. Easy!

Here’s another probable pathway for high glucose levels to lead to poor bone architecture and increased fracture risk. We all ā€œknowā€ that risk factors for osteoporosis and fractures include being thin, and I was taught to assume that heavier folks would be at less risk since the weight of their body put stress on their bones, improving bone remodeling. Interestingly, it turns out that obesity is also a risk factor, at least if it is accompanied by poor glucose control.
And take a note that there were increased fractures even at similar bone densities if the sclerostin levels were high from diabetes. So what does that mean about the predictability of a DEXA scan in a woman with poorly controlled diabetes? Even if the DEXA looks ok, shouldn’t we still be worried?

COMMENT PODCAST for the full show.

A lot of women hear ā€œPCOSā€ and picture one very specific patient:

Overweight.
Acne.
Facial hair.
Hair loss.

But PCOS does not always look like the old textbook picture.

Some women with PCOS are lean. They may look completely healthy from the outside, while dealing with irregular cycles or other signs of hormonal dysfunction.

That is why the diagnosis is not made by appearance alone.

And despite the name, you do not have to have polycystic ovaries on ultrasound to have PCOS.

That was one of the most important points Dr. Wendy Warner made in our conversation.

Sometimes the patient who does not ā€œlook like PCOSā€ is exactly the patient who gets overlooked.

In my early 40s I became a ā€œskinny fat personā€. I had gone through a fairly long stretch of increased stress and, as it turns out, I didn’t do as good a job caring for myself as I thought. At that point in my life, my weight hadn’t changed more than a pound or so here and there since age 15 so I NEVER got on a scale. I just bought the same size clothes and didn’t think about it

But suddenly none of my pants fit—I was losing my waist and developing that weird ā€œback fatā€. I got on the scale and it said the exact same thing it always had. Weird. So I got on the body composition scale…and was horrified. Let’s just say my body fat % was way higher than I wanted it to be, and my basal metabolic rate (how many calories I would burn off if I sat still for 24 hours ) was WAY low, like 1300 calories. Much too low for my height and weight. And, with a bmr that low, I couldn’t possibly ā€œdietā€ my way out of this.

So I really focused on rebalancing my adrenals—I had crashed my DHEA in an attempt to keep my cortisol up, and it led to muscle loss. (When I got on the scale, it was right as the amount of muscle pounds I lost equaled the fat I had put on, so the total looked the same. But fat is bulkier so my clothes didn’t fit). And of course, I also had to work to rebuild the muscle.

Most important was that I did BOTH. Without working to rebalance my adrenals, I would have been chasing my tail and not making progress. Keep that in mind while you’re doing your strength training!

During my training, we talked about FSH only as a pituitary hormone involved in the communication between brain and ovary. Turns out, FSH receptors are on LOTS of tissue, and men make FSH as well (though they don’t have ovaries making follicles). Clearly, this hormone got named wrong!

Recent research is turning up about the other functions of FSH; some is just preliminary but it’s all really fascinating. I’m a coauthor of a new review paper that you might find interesting:

Irregular periods in a teenager are not always abnormal.

But here’s when I start paying closer attention:

Her cycles became regular… and then they became irregular again.

That tells you something changed.

In my conversation with Dr. Wendy Warner, she made a great point:

If the body had already established a rhythm and then lost it, ask what changed upstream?

Stress?

Cortisol?

Nutrition?

Training?

Sleep?

The goal is not just to label the cycle ā€œirregular.ā€

It is to ask:

Why did a previously normal pattern change?

That is where the clinical investigation gets interesting.

Yes, I love new research. I love science.
But can we stop and do the boring stuff? Prioritize sleep. Eat real food. Move our bodies every day. Have a plan to be in Nature every day in some way.
That’s actually a lot.

And once we have that dialed in, maybe we will love our bodies enough to treat them well and stop striving. Stop chasing the next shiny object.

A healthy life, it turns out, isn’t all that sexy.
