I don’t think non-operative management is for everyone, but I believe it’s important for people to know their options by @beyondstrongphysio

🤡 me quietly crashing out 🤡

It’s not that I’m against ACL surgery I’m just bothered by the number of people I see struggling with their post operative rehab (pain, swelling, lack of confidence in the knee) months and months down the line when surgery wasn’t necessary in the first place

Non-operative management is a very viable option even for active individuals (such as myself) and I just hope more people become aware of this

Kind regards,

Yes natural ACL healing is possible and awesomely I get to be proof of that!

My completely ruptured ACL has now reattached and its going to continue to become stronger and more robust over the next couple months

Another finding I didn’t note is “the Sagittal alignment of the knee is maintained”, meaning I’ve healed in a good position without any notable laxity!

The body is pretty capable of amazing things if you look after it

After recently having my own orthopaedic consultation, I realised just how difficult it is to make a life-changing decision in a 15-minute appointment

My recommendation for surgery was based almost entirely on what my MRI showed

What wasn’t discussed was the fact that:
• I have no pain
• I can run at a sub-5:00/km pace
• I can single-leg press 120 kg for 8 reps
• I can walk a slackline
• I have no episodes of instability
• My personal preference is to avoid surgery if it’s safe to do so

None of those factors seemed to influence the conversation. Instead, it felt like the decision had already been made, and I was simply there to hear it

Something else that challenged me was the language used throughout the consultation. Even though I have virtually no functional limitations, I walked away feeling like my knee was “broken, sloppy, unstable” and that surgery was my only realistic option

To be clear, this isn’t about blaming orthopaedic surgeons. Fifteen minutes simply isn’t enough time to explore everything that matters. Imaging is important, but so are your symptoms, your function, your goals, your lifestyle, your sporting demands, and most importantly your own preferences

Surgery is a big decision. It deserves a thorough decision-making process, not just an MRI report

If you’re facing an ACL or meniscus injury, I’d encourage you to educate yourself before your consultation so you can ask better questions and take an active role in the decision

I’ve put together a free guide that walks through the different management options including surgery, rehabilitation, and bracing and explains the pros and cons of each so you can have a more informed discussion with your healthcare team

Not everyone needs ACL surgery and that’s not talked about enough

I know people who would have honestly been better without it, but they were never even given another option

The decision for you to get surgery shouldn’t be decided in a 10 minute consult with an orthopaedic surgeon if they don’t have an understanding of your goals, your lifting history, your functionla and clinical stability, your finances and most impotently your preferences

It is YOUR knee after all

There are options out there other than surgery…

ACLs can heal naturally (yes, even full grade 3 ruptures)

You can return to sport without an ACL (yes, that includes pivotting sport like soccer, basketball and football)

But regardless of whatever option you choose that’s best for you, your rehab is the most important part by @beyondstrongphysio

Returning to sport is just like doing your driving test

If you want to do well you seek out an instructor and they show you what to do and how to do it. Then you practice, practice, practice

Eventually the day comes that both you and your instructor feel like you’re ready do your test or return to sport

If you’ve put the work in it’s likely you pass your test with flying colours or play your first game without any issues

But passing that one test or playing that one game doesn’t guarantee you’ll never be in a crash or get injured in the future. Accidents happen. You may be involved in a car accident entirely out of your control, you may hit a divot in the grass that your knee wasn’t ready for, despite doing everything right

Reason number 2 would be the fact we don’t often drive day-to-day how we do in our driving test. We know what’s right but we often let bad habits creep in, get complacent or let emotions influence our decisions. So despite knowing all the right steps I may have let the passion for the game get the better of me and subsequently put myself in a risky position, which could have been avoided. I was driving too fast due to the thrill of the game and then I crashed…

None of this means rehabilitation doesn’t work

I’m just expressing rehabilitation can’t entirely eliminate the risk of injury

The goal isn’t to make re-injury impossible. The goal is to give yourself the best possible chance of returning safely, performing well, and staying in the game for as long as possible by @beyondstrongphysio

Not all ACL tears need surgery… you’ve just never been told otherwise.

For years, ACL reconstruction has been seen as the “default” treatment after an ACL rupture. But the reality is far more nuanced.

Some people absolutely benefit from surgery.

Others can return to running, the gym, work, and even sport with bracing and/or structured rehabilitation alone.

The best treatment isn’t determined by your MRI, it should be determined by you.

A good decision should consider:
✅ Your MRI findings (including any associated injuries)
✅ Your level of instability
✅ Your sport or activity demands
✅ Your goals
✅ Your lifestyle
✅ Your preferences
✅ Your willingness to commit to rehabilitation

Whether you choose surgery or a non-operative approach, one thing remains the same:

Rehabilitation is the cornerstone of a successful recovery.

There isn’t a one-size-fits-all answer. The best outcome comes from making an informed decision with a healthcare professional who takes the time to understand your individual situation, not just your scan.

💬 Have you been told surgery was your only option after an ACL injury? I’d love to hear about your experience in the comments.

📌 Save this post for later, or share it with someone currently deciding how to manage their ACL injury.

ACL injuries sucks.

And unfortunately the care you receive after can suck even more…..

Too often people are not provided any education or understanding about their injury, how to manage it, their options and what to do next

That’s where I want to change things. Having suffered 2 complete ACL ruptures myself, I’ve got a pretty good understanding of the journey, what you should do and what you shouldn’t

If you’re struggling with an ACL injury or have questions about bracing and natural healing feel free to book a consult with the link below!
