🩺🏁 As #WomenInMedicine #WomenInSurgery we are often “late” to the party of major life events, compared to others who are not in the medical field ! We are later grads, later in the workforce, later to build a family ….
& that’s ok 😄 we are just stretching the fabric of time, after 15+ years of education and training

👯But it can be more insidious & destructive when you compare your journey to other women in medicine, specially to those with higher visibility or leadership role

Don’t compare copy or compete ! We each have our timeline, our priorities & our style in creating a journey of our own- So what if some get “there” faster than you? 🤷🏽‍♀️ You need to get where you need to be !

🎯The most important step is to define what is your most important “why & where” at diffent stages of your life: build a family? Discover the world? Get promoted ? Train for that shiny marathon medal? Take care of your mind & body? …. And go for it , not for what everyone else is doing !

It is your journey & it’s #NeverTooLate you are #NeverTooOld to define & pursue your goals

💡Create your journey ! 🚫 Don’t Compare Copy or Crave someone else’s life

On #NationalDoctorsDay sharing things I wish someone taught me as an early career physician

🩺 Work HARD: yes, I know it feels “old school”, but trust me, work hard to become the master of your craft ! Build a solid foundation as you start defining your career !
🚫 Working hard does not mean exhausting yourself or ignoring other aspects of your life (been there, done that, not recommended !)

🚨Spoiler alert- working hard does not get your promoted & does not advance your career

🏎️That’s why you need to also

🌟Work SMART
📌Tip1 Develop a niche AND diversify your reach within this niche : publish manuscripts, get on your practice committees, become active in GI societies and advocacy …

📌Tip2 Create Visibility and Value for yourself = develop & nurture a professional network and then leverage this network to advance your career
🚨Leadership will not notice your hard work & promote you: put your value on paper and discuss it at regular meetings with your Chief
🚨Don’t limit your networking efforts to those senior to you ⬆️, think about peers & those more junior to you & beyond (nurses, industry partners, staff… ) ↔️ & ⬇️

💎Solid foundation + visibility will allow you to create opportunities for yourself (& for others!) & move your career (not just a job) forward 🏎️

🌼Bonus advice🌼 Medicine is hard - but also so rewarding - if we intentionally keep joy & gratitude at the forefront of what we do ! Take the small & the big wins at work & don’t forget to explore life outside medicine !

#NeverTooYoung #NeverTooOld to kick off your career & achieve work & life integration ! #NoGateKeeping by @dcharabaty

My mindset at the start of 2025 was to Reflect Reassess Reset and Reconnect. The need for that inner work came along as a package deal with the reading glasses & eye creams 😉

I naively thought that it would lead me to kick-start the stable smooth sailing of that “next stage of life”, where everything is supposed to fall into a place of wisdom, content & stability, reaping the benefits of years of stackingup building blocks

🙃Instead, this mindset led to deep quiet decisions & shifts in my professional & personal life; with no splash or drama, like self-evident changes waiting to manifest themselves until I was ready…. Life indeed meets you at your level of audacity & readiness: There’s actual power & strength in recognizing the finite nature of life- & subsequently loosing the fear to start over, change environment, leave what no longer serve you behind, get out of the comfort zone, dream again & re-discover what we aspire to & are capable of & truly need & want

🧧This year it’s all about Protecting my Peace, Optimizing my Health, De-cluttering & Prioritizing whom and what adds Significance, Joy & Prosperity to my life. It might sound selfish, but for me it is an intentional & necessary mindset in my renewed commitment for an aligned life

IBD-related stigma is one of the most significant challenges that hinders open conversations about IBD care.

Dr. Aline Charabaty (@dcharabaty), a gastroenterologist and contributing author of our white paper, “The Power of Community in IBD”, is a champion of creating safe spaces to foster well-rounded IBD care. She emphasizes the role care teams and providers play in destigmatizing IBD and creating supportive environments for patients.

Back2Basics & #IBD Algo
🎯Very early onset IBD #Crohns #UlcerativeColitis #VEOIBD
With #DrDavidLevine @gut.md_ & @gutsglory.md

🧒🏻Definition: Age on onset <6
🔺Make up 15% of all PedsIBD
🧬~15-20% having a monogenic cause — especially infants <age 2
📈 VEO-IBD is on the rise

💎 Dz location: Colon ++ (94%); isolated ileal disease rare
🔬 Histology clues — eosinophilic infiltrates, ⬆️apoptosis; villous blunting/atrophy, ⬆️IEL — can point to monogenic disease
⏰💎All pts should get genetic evaluation

⏰⬇️ Response to TNFi in VEOIBD vs in older kids
✅ IFX 3-yr maintenance 12% VEOIBD vs 67% in REACH trial
💎 Recommendations to ⬆️ réponse to IFX:
✅ Body Surface Area-based dosing (200 mg/m²) + tight TDM
✅🎯 IFX levels: >23 before 2nd infusion, >10 @ maintenance

🧬Monogenic VEOIBD management
💎 Genetic mutation guides targeted therapy, identify CI to drugs
✅ Abatacept for CTLA4/LRBA deficiency
✅ Anti-IL18 for NLRC4
✅ Rapamycin for FOXP3/PIK3CD before HSCT
✅ Hematopoietic stem cell transplant
🚨💎Critical: AVOID TNFi in CGD
🚨💎Hematopoietic stem cell transplant is potentially curative !

🧮 Real #GirlsMath is not a backward logic where women are portrayed as finding “rewards” & “free money” under false premises

🌟🧮🌟#WomenInMedicine Math is empowered 🔀 ; it is all about how to let go of, delegate, automate low rewarding & low paying activities to “buy” time & energy to focus on activities with a higher return on investment

The Morning after ….

What no one tells you in medicine : your grandmother was right - the biological clock is real, but the work clock is not - if you decide to have kids, do it, don’t delay until the “right time” , it doesn’t exist in medicine - but there will always be time to grow your career

In a field focused on health,

reproductive health, pregnancy & #Motherhood can be a a complex & challenging journey for #WomenInMedicine in a system that doesn’t support them

🎙️I recognize my story is not unique, but I am sharing it because:

🤫There’s still so much taboo around #infertility, #IVF, egg freezing, post-partum, women with no children, adoption, single moms … in medicine - as if we, healthcare professionals should be immune to this - in fact, we are at ⬆️ risk of all these realities because of the demands and stress of our career

🤫If we don’t talk about it, if we stay silent & isolated in our own experience then the status quo remains; we need to challenge the system and change the narrative for the next generation of physicians

👩🏽‍⚕️I wish someone reminded me early on, that the biological clock is real; and I now know there is NO career clock (despite what others may convince you of and the pressure to put personal plans on hold to keep the career “going”); if you choose to have kids, plan it, do it, don’t postpone it; partner or no partner - I assure you, you can take a career time out & jump right back into the race & excel doing it 🏎️🏎️🏎️

🔥We got the power ! You are NeverTooYoung #NeverTooOld to negotiate & advocate for yourself or others, for fair parental leave , flexible work schedule, path of re-entry into the work force

💙It’s time we normalize the fact that women in medicine can have kids , or may choose not to have any, & whatever personal journey they are walking, they are phenomenal physicians & amazing human beings by @dcharabaty
