IBD-related stigma is one of the most significant challenges that hinders open conversations about IBD care.

Dr. Aline Charabaty (@dcharabaty), a gastroenterologist and contributing author of our white paper, “The Power of Community in IBD”, is a champion of creating safe spaces to foster well-rounded IBD care. She emphasizes the role care teams and providers play in destigmatizing IBD and creating supportive environments for patients.

Back2Basics & #IBD Algo
🎯Very early onset IBD #Crohns #UlcerativeColitis #VEOIBD
With #DrDavidLevine @gut.md_ & @gutsglory.md

🧒🏻Definition: Age on onset <6
🔺Make up 15% of all PedsIBD
🧬~15-20% having a monogenic cause — especially infants <age 2
📈 VEO-IBD is on the rise

💎 Dz location: Colon ++ (94%); isolated ileal disease rare
🔬 Histology clues — eosinophilic infiltrates, ⬆️apoptosis; villous blunting/atrophy, ⬆️IEL — can point to monogenic disease
⏰💎All pts should get genetic evaluation

⏰⬇️ Response to TNFi in VEOIBD vs in older kids
✅ IFX 3-yr maintenance 12% VEOIBD vs 67% in REACH trial
💎 Recommendations to ⬆️ réponse to IFX:
✅ Body Surface Area-based dosing (200 mg/m²) + tight TDM
✅🎯 IFX levels: >23 before 2nd infusion, >10 @ maintenance

🧬Monogenic VEOIBD management
💎 Genetic mutation guides targeted therapy, identify CI to drugs
✅ Abatacept for CTLA4/LRBA deficiency
✅ Anti-IL18 for NLRC4
✅ Rapamycin for FOXP3/PIK3CD before HSCT
✅ Hematopoietic stem cell transplant
🚨💎Critical: AVOID TNFi in CGD
🚨💎Hematopoietic stem cell transplant is potentially curative !

Thought-provoking insights and questions during the #MondayNightIBD convo on #ASUC Acute Severe #UlcerativeColitis, led by @joshsteinbergmd

Masterfully summarized in this #IBDAlgorithm by @gutsglory.md

❓Why are we still waiting for IV corticosteroids failure on day 3 to start effective “rescue” therapy , when we know the patient will need an advanced therapy

❓Will JAKi (TOFA & UPA) replace TNFi as rescue therapy for ASUC: lower cost , easier to access in 🏥 & after discharge

❓Can we manage select patients with ASUC outside the 🏥 or in a HospitalAtHome setting with close monitoring

🔺Summary of guidelines in the algo, with a look into the future of ASUC managment
✔️IFX still most commonly used rescue therapy
✔️Choose JAKi if low albumin, no response or loss of response to IFX prior to ASUC
✔️Accessibility & cost might start favoring JAKi > IFX
✔️Consult surgery early on admission, & definitely if no response to IVCS on day 3 - ⚠️If patient needs surgery, don’t delay it ! Delay in surgery associated with increased post-op complications !

🌟#PatientExperience convo on the patient surgical journey led by @loriplung : what can clinicians do to help patients make the decision for surgery when they need it : ✔️Talk about surgery early & not as a “last resort” option, but as the best treatment option for the current situation ✔️Validate patients fears and answer patients questions over more than one visit if needed ✔️Explain to patient what to expect during post-op recovery and post surgical QOL ✔️ Connect the patient with a peer / advocate who had similar surgery (a great resource shared by Lori is the Power of 2 by @crohnscolitisfoundation ) by @mondaynightibd

🚨Acute Severe Ulcerative Colitis (ASUC) is a medical & surgical emergency 🚨ACT FAST

📌Admission, evaluation & therapies have to be done in a very timely manner
📌Patients’ condition can deteriorate quickly and require emergent surgery: monitor clinically + by labs + have the surgical team on board from the time of admission

🔥#MondayNightIBD #Back2Basics 🔥 by Drs Ifrah Fatima & Aimen Khan

✅Recognize ASUC -> 🏥
✅DO’s: Labs to order, stool for CDiff, flex sig with CM biopsie, DVT prophylaxis- Steroids & rescue therapy- Surgical consults
🚫DON’T: antibiotics, TPN, narcotics …

‼️CDiff doesn’t cause pseudomembranes in IBD patients: don’t rely on flex sig to make the diagnosis, check stool for CDiff

‼️If CDIFF + or CMV +: Treat the infection AND underlying ASUC
⚠️These patients are at higher risk of needing a colectomy ✂️

⏳If BM, blood, CRP do not ⬇️ by > 50% on day 3 of IV steroids -> Start Rescue therapy : IFX, CysA, JAKi, or Surgery

🔮Check predictors of steroids & IFX failure

💉IFX optimization (10mg/kg) & acceleration (infusions at 5 days interval x2-3) in ASUC: short-term benefit in select patients with high CRP, low albumin

✂️DONT delay surgery in the patient who needs it ! Delays = ⬆️ risk of op & post-op complications

💎#IBDAlgo by @gutsglory.md summarizing key point of the 💥convo led by @jordan_axelrad :

Approaching IBD therapy in a patient with #IBD , a new diagnosis of #cancer in need of immunotherapy

⚖️Optimize IBD control & minimize systemic immunosuppression & impact of IBD therapy on cancer progression
✅Preference goes to Vedolizumab & anti-IL23, but if anti-TNF is needed as a MOA it should be considered

🚫Which therapy to stop by type of cancer?
🔺Lymphoma: ✋🏽thiopurines & anti-TNF
🔺Other heme malignancies: ✋🏽 thiopurines
🔺Melanoma: ✋🏽anti-TNF
🔺Recurrent SCC: ✋🏽 thiopurines
🔺Solid organ CA: ✋🏽thiopurines if cervical or urinary tract CA

✅Otherwise: OK to continue other biologics (Vedo, UST, IL23)

💉Effect of cancer therapies on IBD
🔺Cytotoxic chemotherapy
⚠️can put IBD in remission for few years (and ok to hold off IBD immunosuppressants )
🔺#Immunotherapy/ Immune check point inhibitor
⚠️can lead to ICI-colitis (that mimics IBD) & risk is higher in patients with IBD
⚠️treat early with IFX or Vedo (similar efficacy; Vedo might have an advantage on progression free/ overall survival)
⚠️High dose steroids at ICI-colitis dx can also decrease progression free/ overall survival

📣Patients with IBD: wear sunscreen & get an annual total body skin check : IBD increase the risk of melanoma regardless of therapy

📣MDs: enroll patients with IBD & CA in the SAPPHIRE registry so we can better understand how to care for our patients

Kicking off 2026 with a #Back2Basics review of #IBD & #cancer @daniyal8540 @gutsglory.md

🔺IBD inflammation-driven GI malignancies
✔️Colon cancer in colonic disease involving more than 1/3 of the colon (start colonoscopy surveillance after 7-8yrs of IBD; start at diagnosis in IBD+PSC)
✔️Small bowel cancer in SB crohns
✔️Anal cancer in perianal crohns : anal squamous cell & mucinous adenoCA that can arise in fistula ⚠️
✔️CholangioCA in PSC

🔺IBD therapies-associated malignancies
✔️Thiopurines→ lymphoma, urinary CA (men > age 50), NMSC
✔️Anti-TNF mono → low risk of lymphoma
✔️Combo thiopurines+ anti-TNF → highest risk for lymphoma
✔️JAKi: signal for lymphoma/lung/NMSC
✔️5ASA, Vedo : no malignancy risk
✔️IL23: no signal so far

🔺Knowledge Pearls
💎Relative risk ≠ absolute risk: when a malignancy is rare in the general population, it still remains uncommon in patients at higher risk
💎Risk of lymphoma is mainly driven by thiopurines (& increased by adding anti-TNF)
💎Most common type of lymphoma w thiopurine is EBV-associated post-transplant like: ⬆️ risk after >1-2 yrs of use, mainly in men > age 50, risk goes back down to that of general population after discontinuation
👉🏽 avoid use in men > age 50; stop it after 1-2 yrs of use in patients on combo anti-TNF + thiopurines if possible (⚖️ risks & benefits of combo therapy)
💎Learn about the other 2 rare (but poor prognosis) types of thiopurines-associated lymphomas in men < age 35 (in slides)
💎IBD patients are at higher risk of melanoma, regardless of therapies ➡️ use sunscreen 🧴& annual skin check
