Most board forecasts are written so that no outcome can falsify them. This is rarely deliberate. It is what happens when nobody is required to be specific and being specific carries risk.

The fix is mechanical. Four components, and a rule about what happens afterwards.

Between 1984 and 2003 Philip Tetlock collected roughly 28,000 probabilistic forecasts from 284 experts on political and economic questions, then scored every one against what actually happened.

The aggregate result was that experts performed barely better than crude statistical baselines. The finding that should concern any board is the second one. The most confident and most publicly visible forecasters were among the worst calibrated.

The correction is more useful than the diagnosis. In the forecasting tournament run for IARPA from 2011, Tetlock and Mellers identified a group who substantially outperformed everyone else, including intelligence analysts with access to classified material. Their advantage was not access, seniority or subject expertise.

It was method. They gave numerical probabilities rather than adjectives. They set resolution dates. They updated in small increments when evidence arrived rather than defending a position. They kept score.

Most institutional forecasting fails the first requirement. A prediction expressed as "likely" cannot be scored, which means it cannot be wrong, which means nobody improves.

In 2007 Nokia held roughly half the global smartphone market. Within five years the business was effectively finished. The standard account is complacency, or a weaker operating system, or missing the touchscreen.

Vuori and Huy interviewed Nokia managers across the 2005 to 2010 period and published the findings in Administrative Science Quarterly. What they describe is not a technology failure.

Senior managers were afraid of competitors and shareholders. Middle managers were afraid of senior managers. The second fear is the one that did the damage.

Middle managers who understood how far behind the software was did not say so, or said it quietly, or said it in terms soft enough to be survivable. Some overstated what was achievable because the alternative was to deliver news that carried a personal cost. Senior managers, receiving optimistic reports, saw no reason to change course.

The information existed inside the organisation the entire time. It was accurate. It never arrived.

Nokia did not have a strategy problem in 2007. It had a reporting problem that produced a strategy problem.

A number that has been cited a thousand times has been checked once.

Usually by nobody.

Monday made the case that a thirty-year-old statistic entered professional use without a source. The obvious follow-up question is what to do about the papers already in front of you.

Ninety seconds per claim is enough. Not to verify it, but to find out whether anyone has.

The point of these questions is not to catch people out. It is that asking them once, publicly, changes what gets written in the next pack. Papers are drafted against an anticipated audience. If the audience is known to ask where a number came from, numbers arrive with sources.

That change costs one meeting. by @mindfriendpro

Institutions do not lose trust in a single event.

They lose it in the gap between what was promised and what was practised.

Trust is not a single quantity that goes up or down. It has components, and they fail separately.

Treating a competence failure as though it were an integrity failure produces an apology where a plan was needed. Treating an integrity failure as though it were a competence failure produces a plan where an account was needed.

Most institutional recovery efforts fail on this diagnosis rather than on effort.

Trust is routinely filed under culture, which is to say it is routinely filed under things that are discussed but not managed.

The economic literature treats it differently. Trust reduces transaction costs. Low-trust environments require more verification, more contracting, more escalation and more time to reach the same decision. Every one of those is a line item.

Inside organisations the effect shows up most sharply in information flow. The research on error reporting is consistent. Teams that report more problems are not teams with more problems. They are teams where reporting one does not carry a cost.

Which means an institution with a clean incident log has one of two things. A well-run operation, or a workforce that has correctly assessed the consequences of speaking.

Distinguishing between those two is one of the more valuable diagnostic exercises a board can commission, and one of the least often requested.

Every incentive is a hypothesis about human behaviour.

Every incentive is a claim about how people will respond. Most are introduced without anyone testing that claim.

Five questions that surface the second-order effect before it arrives in the results.

Goodhart put it plainly. When a measure becomes a target, it ceases to be a good measure.

The behavioural mechanism underneath it is better documented than the phrase. Holmstrom and Milgrom showed that when a role has multiple dimensions and only some are measurable, incentivising the measurable ones causes effort to move away from the unmeasured ones. Not through dishonesty. Through rational response to a stated priority.

This is why a target on call handling time degrades the quality of the call. Why a target on units shipped degrades the inspection that precedes it. Why a target on new client acquisition degrades the servicing of existing ones.

None of these outcomes require anyone to behave badly. They require only that people behave in the direction they were pointed.

The failure is upstream. Someone chose a measure because it was available, not because it captured what mattered, and the organisation optimised faithfully towards it.
