If you’ve received a prostatectomy, the surrounding tissue (bladder, bowel, muscles, ligaments) can be subjected to scar tissue. Imagine an ACL surgery or similar procedure to fix a broken bone. Surgery to remove the prostate leaves behind the same dense collagen fibers that form scar tissue at the excision site.

But because the prostate is so close to the bladder and rectum, scar tissue left behind from a prostatectomy can actually influence bladder, bowel, and pelvic floor control.

Unique to our profession is the ability to directly treat scar tissue with internal manual therapy. By increasing blood flow, relaxing those taut collagen fibers, and retraining muscular coordination, pelvic floor therapy supports your recovery, helping tissue return to its capability of expanding and contracting to allow for better bladder control.

This is why we believe pelvic floor therapy is essential after a prostatectomy.

It’s Men’s Health Month!

Pelvic Floor Dysfunction in males is more common than you might think — the muscles at the base of the pelvis, when too tight or too weak, can cause or contribute to many common concerns in male pelvic health.

Pelvic Floor Therapy is not just for women! The right pelvic floor therapist will use a combination of techniques to treat your pain and adverse symptoms, including internal “mapping” of the pelvic floor muscles, maintaining proper abdominal expansion, and retraining the pelvic floor muscles to promote optimal function.

If you experience any of these conditions, Pelvic Floor Therapy can help. Book online any time at tonicandphasic.com.

Diastasis Recti Abdominis (DRA) is one of the most common issues people notice during and after pregnancy.

Do you have DRA? Most likely, yes. By the third trimester, nearly everyone develops some degree of separation through the abdominal wall. Your body is expanding to make space for a growing baby, and the connective tissue between your abdominal muscles stretches to allow that to happen. This isn’t a problem - it’s expected!

So postpartum, if you’re feeling a gap, noticing a change in your abdomen, or feeling less supported, that’s a very normal place to start and you’re not alone.

Once we assess for DRA in Pelvic Floor therapy, we treat it by rebuilding your foundation of breathwork, abdominal coordination, and progressive load, teaching the system how to turn on again and retraining our abdominal coordination.

Often, we either don’t load the core enough or we push past abdominal control. The goal of a Pelvic Floor Therapist is a guided return to core strength.

You got this! Learn more about DRA at tonicandphasic.com/blog

Part Two: Pessaries!

One of our most-used tools for supporting the pelvic organs in prolapse. Pessaries come in different shapes and sizes, depending on what your body needs — and when we use them, we can conservatively treat the surrounding tissues for better muscle coordination, strength training, and pressure mechanics.

Dr. Allea Francis, PT, DPT, is certified in pessary fittings. To learn more, call us at (949) 612-7695 or visit our website, tonicandphasic.com.

UTI symptoms but your tests keep coming back negative?
You’re not imagining it—and you’re definitely not alone.

Bladder urgency, burning, and frequent urination can come from pelvic floor tension, nervous system dysregulation, hormones, constipation, and more.

In our newest blog we break down the hidden reasons behind bladder pain and what you can actually do about it.

If you’ve ever been told “everything looks normal” but you still feel miserable, this one is for you.

She went 10 years in chronic pelvic pain.

Doctors. Medications. Procedures. Nothing helped.

Then she discovered pelvic floor physical therapy — and has been pain-free (and helping other women achieve the same) ever since.

Today, @thepelvicptdoc joins us to talk about the conversation most of us were never given about our own bodies.

Leakage. Back pain. Painful sex. Heaviness. Constipation. Hip pain. Hormonal shifts. The glute you didn’t know you weren’t firing.

All of it connected. All of it treatable.

Today is for every woman who has been told something is just part of being a woman. For the one dismissing small symptoms because they don’t feel dramatic enough.

New episode of Mighty as a Mother is live now!

What’s one thing about your body you’ve been quietly ignoring? Tell us below. ⬇️

Your back pain might not be a back problem.

Most of us were never taught what the pelvic floor actually does. Or how many symptoms connect back to it. Or that back pain, hip pain, and sciatic pain can have nothing to do with your spine.

This week, we sit down with @thepelvicptdoc — pelvic floor physical therapist and co-founder of @tonicandphasic — for a conversation that changes the way you think about your body.

The symptoms nobody connects. The muscles most moms are quietly neglecting. And why “common” and “inevitable” are not the same thing.

You do not have to be symptomatic to need this information!

Save this and share with a woman who deserves to feel good in her body.

New episode drops tomorrow. 💕

Nerves and the nervous system are not just passive wires—they actively respond to stress, inflammation, and repeated irritation.

When a nerve root is irritated, the surrounding tissues and nervous system can become more sensitive over time. But just as it can become more sensitive, it can also become less sensitive again with the right support, movement, and gradual exposure to activity.

Improving hip strength, restoring natural movement, calming sensitive nerves, and supporting the nervous system can all play a role in helping symptoms improve.

And for many people, these changes slowly allow the body to regain confidence, resilience, and comfort again. 🤍

Dr. Allea, PT, DPT’s set for sciatic nerve pain/symptoms.

Dr. Allea shows us a few exercises that help make room for our sciatic nerve and increase bloodflow to this area!

If the hips are stiff, weak, or irritated, the body compensates: the lower back takes on more load than it should, glute muscles begin to compress nerves, and pain gets referred into the buttock or thigh.

And when the pelvic floor is tense, guarded, or fatigued, it can create additional pressure on the pelvic nerves, even changing the way our pelvis and hips move.

All of these stretches help to target these contributing factors to sciatica: more blood flow to the pelvis and more lengthening of these tissues allows for the nerves to move freely (which can help manage sciatica).

Comment “Nerve” for a link to our post all about Pudendal Neuralgia!

Dr. Allea Francis, PT, DPT:

The pudendal nerve comes through the spine in the sacral nerve plexus, innervating many structures, like the external genitalia, pelvic floor, and perineal muscles. When it’s irritated, many people mention:

* burning or stabbing pain
* electric or shock-like sensations
* tingling or numbness
* a feeling like something is stuck in the vagina or rectum
* urinary urgency or frequency
* pain during bowel movements with quick relief afterward
* pain during or after clitoral orgasm or ejaculation

An irritated nerve is a bit like a bruise: if something continues to irritate it (like tight pelvic floor muscles, the wrong exercises, or even stress), it won’t have the right environment to heal. A pelvic floor therapist’s job is to provide that environment for recovery, working with you to reduce pressure, relieve tension, and regulate the nervous system.

Nerves are delicate structures—they do not like to be overstretched or compressed. Healing takes time but your nerves are capable with the right environment. <3
