✨ NEW PUBLICATION IN JEADV! ✨ Lichen sclerosus is more than a skin condition. It is a disease we need to talk about. 💜 I am delighted to share our newly published review, “Female genital lichen sclerosus,” in the Journal of the European Academy of Dermatology and Venereology (JEADV), co-authored with Ashley Spencer. 🔬 Why does lichen sclerosus awareness matter? Female genital lichen sclerosus is a chronic inflammatory disease that can profoundly affect women’s quality of life, sexual health and vulvar anatomy. Importantly, it is also associated with an increased risk of vulvar squamous cell carcinoma. Early recognition, effective treatment and long-term surveillance are essential. 💡 What does the future hold? Beyond established treatments such as ultrapotent topical corticosteroids, emerging research is exploring new therapeutic approaches, including off-label JAK inhibitors for selected, treatment-refractory cases. These therapies are promising research directions, although clinical evidence remains limited and their use requires specialist supervision. Our review explores: * 🔬 Immunological mechanisms and disease pathogenesis * 🩺 Diagnosis, clinical management and cancer surveillance * 💊 Emerging targeted and off-label therapies * 🧬 Biomarkers, innovative diagnostics and future research directions More awareness. Mild HS — what do the European S2k Guidelines recommend? 🔎 For mild active HS (IHS4 1–3), oral tetracyclines are a key recommended treatment, typically given for 3 months. Imaging in Hidradenitis Suppurativa: What Does the Evidence Show? Very happy to share our new review published in JEADV: A key contribution of this review is the comparison of the level of evidence supporting different imaging techniques in HS: 🔹 ULTRASOUND Diagnosis: Level I (A) Severity/activity: Level I–II (A–B) Pre-surgical mapping: Level I (A) Image-guided treatment: Level I (A) 🔹 MRI: Level IV–V (D–E) for diagnosis, staging and pre-surgical mapping 🔹 Dermoscopy: Level IV–V (D–E) for diagnosis 🔹 Medical Infrared Thermography: Level IV–V (D–E) for pre-surgical mapping 🔹 OCT & Reflectance Confocal Microscopy: Level V (E) for diagnosis 🔹 CT & PET-CT: evidence not available for these evaluated applications. 📌 Ultrasound currently has the strongest evidence among imaging modalities in HS, supported by validated diagnostic criteria and scoring systems, multicenter studies and international consensus. 🤖 AI is promising, particularly for early detection and severity assessment, but further prospective multicenter validation is needed. “Clinically relevant evidence on imaging and artificial intelligence in hidradenitis suppurativa” Wortsman X, Boostani M, Kiss N, Dini V, Ioannidis D.
